In process of removing 14-10 miner head from miner, employees were working on ripper chain to remove chain pins. They used a slate bar and come-alongs to loosen chain. After pins were pulled with slate bar in chain the back part of chain was fouled against the slate bar. Miner man let head down to tram forward, when slate bar shot out of chain hitting employee.

EE WAS BOLTING TOP WITH A ROOF BOLTER WHILE PUTTING HIS STEEL TOGETHER, HE CAUGHT HIS LEFT INDEX FINGER BEWEEN STEEL, RIPPING A PIECE OF SKIN AND MEAT FROM HIS INDEX FINGER ON HIS LEFT HAND.
