WHILE TRAMMING THE MINER, THE OPEATOR MOVED HIS HEAD OVER THE MINER FRAME WHILE LOOKING FOR CABLES WHICH MIGHT BE DAMMED AS HE TRAMMED. AS HIS HEAD WAS BETWEEN THE MINER FRAME AND MINE ROOF, THE MINER RAN OVER A LARGE ROCK CAUSING THE MINER TO LIFT UPWARD. THIS UPWARD MOTION PINCHED HIS HEAD BETWEEN MINER FRAME AND ROOF, WHICH CUT HIS EAR.

WHILE HE WAS TRAMMING ROOF BOLTER HE LAID HAND ON TOP OF MACHINE. BOLTER RAN OVER ROCK WHICH CAUSED MACHINE TO MOVE UPWARD. MOTION CAUSED HAND TO BE CAUGHT BETWEEN ROOF & MACHINE. THIS CUT HIS FOREFINGER.

EMPLOYEE WAS DR3LLING A BOLT HOLE IN THE ROOF IN PREPARATION TO INSTALL A 48" CONVENTIONAL ROOF BOLT AND 60" METAL STRAP. HE DRILLED TOO CLOSE TO A KETTLE BOTTOM WHICH CAUSED A PORTION OF THE ROCK TO BREAK 5ND FALL,STRIKING THE EMPLOYEE.
