EMPLOYEE WAS OPERATING A D9L DOZER THE DOZER DROPPED INTO A HOLE ABOUT 4 FEET DEEP CAUSING EMPLOYEE TO STRAIN HIS NECK AT THE TIME EMPLOYEE COMPLAINED OF A SORE NECK BUT DID NOT THINK HE NEED ED TO SEE A DR HIS NECK HAS APPARENTLY BOTHERED HIM OFF AND ON SINCE EMPLOYEE WENT TO SEE A CHIROPRACTIC DR ON 5-19-88 AND FILED A WORKMENS COMP CLAIM ON 5-10-88
