The bolt sheared off when the miner was applying torque and the miner lost his balance and fell, striking his shoulder. Foreman was training an hourly employee on moving a Joy, 12/15CM miner. During this process the foreman inadvertently entered the red zone and was struck, on the hip, by the CM boom. Rules, Regs. and equipment were a factor. The following were not a factor: skills, proficiency, training, attitude, clothing or protective devices on equipment.
The employee was operating the head-gate side of the shearer, mining toward the tail-gate. The miner stopped the shearer and walked back ten shields toward the head to remove an obstruction. The pontoon of a shield pushed his leg against the pan-line. Violating company rules was a factor. Not a factor: Regulations, equipment, protective devices, attitude, skills, or clothing.
Employee was roof bolting drilling a bolt hole. His right hand was resting on the drill steel tray and 1 drill which put into the tray was going up with drill head. The drill steel slipped off the raising head and fell striking his index finger. Began losing time on 5/8/2014
Employee had finished filling a rockdust tank. He thought the pressure was off the hose and released the hose. Pressure with rockdust hit him in the face and ear.
A mechanical pod rock duster was started and the injured walked near the end of the rock dust hose. A plug had not been removed from the hose and blew off striking a roof board and then striking the injured hand. Stitches.
Employee was checking on a scoop tractor. The section was moving power and the miner cable was pulled on inby pulling the miner cable down on the injured neck.
Employee had finished changing a dust suction on the roof bolter. Asked the drill to be started and the head started to raise slowly pinching his fingers between the long and short arm. The up lever was partially engaged to cause the head to raise. 2 fractured fingers
Employee was roof bolting. A drill became stuck in the roof. He went to check his suction and the steel fell out of the roof striking his hand.
Employee was operating a roof bolter installing a roof bolt. The bolt wrench slipped off the roof bolt head and struck his right hand.
The employee was assisting with screening the longwall face. The injured needed to remove a hook from the roll of screen. The employee assumed the shield was stopped and stuck his arm in the shield and got it caught on the shield.

Employee was assisting another roof bolter to install messenger wire. A 4 foot roof bolt was being installed to secure the messenger wire and the injured was holding the messenger wire in place. The roof bolt bent causing the messenger wire to lacerate the injured's middle finger. Avulsion of the tip of the finger.
Employee was roof bolting. His steels became clogged dropped the head and spun the head. Steel hit him above the left eye, 10 stitches.
EE was roof bolting he was installing a cable bolt. It became stuck and turned the rotation on and the bolt struck his left wrist.
EE was changing bit on drill steel. The steel was still in drill head and his right hand was on top of drill steel when he slipped and struck the upfield on the valve bank which drove drill steel into his hand against the roof.
Employee was jacking rail car onto rails when handle of jack slipped out of hands and struck him in chest. Started to lose time 12-30-11.
EE was roof bolting. Was setting and swinging his boom into position. His right hand was on a holder under the boom. The canopy struck the rib, a piece of stone was protruding from the rib. Hand pinched between rib and metal handle.
EE was attempting to open a void in the clean coal pile when the dozer fell into the void and was entrapped for over 30 minutes.
Employee was checking a shield. He thought he had disabled the shield. The shield move in and bruised his ribs.
EE was using a torch when an actylene hose broke behind the blow back valve causing burns to the right hand.

EE was lowering a canopy with his hand on the canopy stack when his right index finger was pulled down into the stack causing a severe laceration on the tip of his finger.
Employee was lowering the drill head with his hand resting on the boom when his right middle finger was pinched between the drill head and the roof bolt tray. He received 6 stitches in his finger.
Employee was operating continuous miner when the cutter head caught a loose piece of metal in the mine floor. The piece of metal flew back and struck his left leg. The employee received 20 stitches in his leg.
EE was operating a roof bolter installing supplemental roof supports (12 feet long cable bolts) in an area outby the face areas. As he was using boom of the bolting machine to push the bolt into the hole, bolt caught in the hole bent and rotated, the bolt struck the EE in his right upper arm.
EE was standing on top of roof bolter trailing cable to help unwind the cable as the machine trammed forward, cable pulled tight and caused EE to fall.EE hit his head on the bucket of a scoop car and lacerated his head. 2inches long and located above left eye, 6 staples to close cut.
Employee was operating the tailgate end of the longwall shearer on the. 3 west off 3 cool hill longwall section. A stone was thrown by the rotating drum of the shearer. The stone struck the ee on his left hand. His left small finger was fractured. He was wearing metacarpal gloves stone was the size of a football. He was standing by shearer body.
Employee was assisting in lowering the stack of the ATRS jack. The section of the stack collapsed & smashed his left finger between the stops of the section.
An EE was installing a 4' bolt hanging the monorail, using an air powered Cobra drill. As he was pushing the bolt into the bolt hole, it got hung up and bent the bolt, causing the drill chuck to shoot upwards, striking the EE between the eyes. Broke his safety glasses, lacerating the bridge of his nose and the orbit area of his right eye.
Bolting up plant in track entry of coal hill. Drilling outside hole operator side. Reaching drill steel, look at his buddy as the plank kicked. Drill steel came out of the roof and struck his finger.
Installing roof bolt, when straightening bolt, the bolt twisted with bend in bolt. It pulled employee to the point he fell to the ground. At this time he felt strain in his shoulder & back.
Rock duster off track and using air bags to lift blockings. Dusted, then tried to straighten tracks. Duster slid about 6 inches, catching right leg between ankle and knee.
While operating a drill to bolt the longwall face, the rotation stuck on the drill when tightening a bolt. The drill swung around and struck him in the right rib.

EE WAS ASSISTING WITH RIB BOLTING. AS HE WAS PUTTING A ROOF BOLT THROUGHT THE T-5 CHANNEL, THE CHANNEL SLID BACK, PINCHING HIS LEFT INDEX FINGER.
An employee was roof bolting (with cobra drill) on the 21 west long recovery face, a piece of stone 28" long by 7" w by 8" fell between the screen and the tip of shield, striking him on the top of his right hand. Fx metacarpal bone near ring finger.
EE WAS STANDING IN FRONT OF PARKED C/M, HANGING THE REMOTE CONTROL BOX FROM ROOF. A ROOF BOLTING MACHING WAS MOVING ACROSS THE SECTION. ITS TRAILING CABLE WAS LODGED BEHIND THE WATER BOX & WOULD NOT ALLOW THE CABLE TO UNWIND. CABLE TIGHTENED & JERKED FROM THE HOOKS IN ROOF. CABLE STRUCK EE ON THE R. SIDE OF HEAD & KNOCKED HIM TO MINE FLOOR. EE WAS UNCONSCIOUS APPROX. 12 HRS.
EE WAS USING A Z DRILL, ROOF BOLTING THE TOP ON THE LONGWALL FACE. DRILL STEEL GOT CAUGHT IN THE HOLE, TWISTING THE DRILL AND PULLING EE'S BODY SIDEWAYS, STRAINING HIS LOWER BACK.
EMPLOYEE WAS DRILLING A BOLT HOLE WHEN A PIECE OF ROOF ROCK BROKE LOOSE, SLID DOWN THE ROTATING DRILL STEEL, BROKE INTO PIECES WHICH FLEW & STRUCK HIS CHIN, LACERATING THE LOWER LIP.
THE EE WAS ROOF BOLTING AT THE BOTTOM PROJECT BELT OVERCAST TRENCH, WHEN A PIECE OF ROOF SLATE FELL STRIKING HIM ON THE HEAD, PUSHING HIS HARD HAT DOWN ON HIS SAFETY GLASSES, LACERATING AND FR ACTURING HIS RIGHT CHEEKBONE.
EMPLOYEE WAS HELPING OTHERS INSTALL A MECHANICAL SPLICE IN A BELT. AS HE WAS OPERATING THE DEVISE THAT INSTALLS THE CLIPS IN THE BELT SPLICE HE FELT PAIN IN HIS BACK. THE DEVICE IS OPERATED WI TH HIS HANDS. INPROPER BODY POSITION CAUSED THE INJURY. THERE WAS NO DAMAGE OR IMPAIRMENT TO THE OPERATION OR TO ANY EQUIPMENT. NO OTHER FACTORS WERE INVOLVED.
EE WAS OPERATING A ROOF BOLTING MACHINE AT THE FACE AREA. HE HAD DRILLED A BOLT HOLE AND WAS ATTEMPTING TO PUSH A BOLT ASSEMBLY INTO THE HOLE. HE BOLT CAUGHT ON THE SIDE ON THE HOLE AND STOPPE D SUDDENLY. EE FELT PAIN IN BACK. THE WAS NO DAMAGE, IMPROVEMENT OR OTHER FACTORS INVOLVED.
EE WAS OPERATING A LONGWALL SHEARER. THE ROTATING DRUM OF THE SHEARER THREW A PIECE OF STONE WHICH LACERATED EE'S LT. INDEX FINGER. 4 SUTURESWERE REQUIRED TO CLOSE WOUND. EE WAS NOT WEARING GLOVES WHEN THE ACCIDENT OCCURRED. NO OTHER FACTORS WERE INVOLVED.
EE WAS OPERATING ROOF BOLTING MACHINE HE WAS SPINNING A 12FT CABLE BOLT WITH BOOM ROTATION FOR INSTALLATION. HE HAD HIS HAND ON THE BOLT WRENCHWHILE SPINNING THE BOLT. HIS GLOVE BECAME WRAPPE D AROUND THE BOLT WRENCH.HIS LEFT RING FINGER WAS TORN OFF AT THE MIDDLE KNUCKLE. HIS LEFT SMALL FINGER WAS FRACTURED. HE SHOULD NOT HAVE HAD HIS HANDS ON BOLT WRENCH
EE WAS OPERATING A ROOF BOLTING MACHINE. HE WAS DRILLING A HOLE FOR ROOF BOLT INSTALLATION. AS HE WAS DRILLING A STONE FELL FROM THE ROOF AND STRUCK THE BOOM OF THE MACHINE. A HYDRAULIC FITTIN G WAS BROKEN. THE BOOM FELL STRIKING THE EE'S RIGHT SHOULDER. NO OTHER FACTORS INVOLVED.
EE WAS MOVING THE CONTINUOUS MINING MACHINE FROM THE FACE OF #1 ENTRY. AS THE MACHINE WAS BEING BACKED INTO THE CROSSCUT FROM 1 TO #2 ENTRIES. THE BOOM ON THE MACHINE, PINNED THE EE AGAINST TH E RIBLINE, FRACTURING HIS PELVIC AREA. THE REMOTE POWER CORD WAS CUT BY THE BOOM AND THE MACHINE SHUT DOWN. THE EE WAS STANDING TO CLOSE TO THE MACHINE WHILE OPERATING IT REMOTE CONTROLLED.
EMPLOYEE WAS USING ON SHIELD CONTROLS TO LOWER A SHIELD (150) FROM THE TOP. WHEN THE SHIELD WAS LOWERED IT ADVANCED ITSELF DUE TO A STUCK SOLONOID. WHEN THE SHIELD ADVANCED IT PINCHED HIM AGAI NST THE PANLINE, BRUISING HIS CHEST AND BACK. NODAMAGE OR IMPAIRMENT OCCURRED TO THE MINING OPER OR TO THE EQUIPMENT. EE SHOULD HAVE CHECKED TO OTHER CONTROLS BEFORE LOWERING SHIELD. JOB SKIL
