Showing all 8Employee had been working on cutting out deck on vibrator. While he was going in and around deck to cut bolts he strained his back. Employee did not report accident until 05/19/2014, he worked 5/20 and 5/21 then went to the doctor which scheduled him off work for 5/22 and 5/23. Due to holidays there was no work scheduled for this employee until 5/26/14.
EMPLOYEE WAS HELPING LOADING EQUIPMENT( A VULCANIZING PRESS) ONTO OUR TRUCK. THE PRESS WAS 3'OFF THE GROUND WHEN THE EMPLOYEE WAS GUIDING THE PRESS AWAY FROM THE RAILING TO PREVENT FROM HITTING WHEN THE STRAP BROKE .THE PRESS CAME DOWN THE Employer's LEGS THEN LANDING ON BOTH ANKLES AND FEET.
An old beech tree log was laying on the ground. Employee was cutting brush around log when it started to roll and rolled over on him, trapping him under the log.
Employee was watching another employee excavate a footer, to watch for a ground cable and conduit, when the backhoe bucket struck him knocking him down. He struck his chin on something causing a laceration.
Excavator rolled off work bench when out slope failed. Employee suffered broken ribs and lacerations.
EMPLOYEE REPORTED SEVERE BACK PAIN WHILE OPERATING A BULLDOZER.
RUST FELL OFF VIBRATOR PAN AND CUT RIGHT HAND.
