EE stated that he was collapsing the head of the bolt machine when he caught his forearm in a pinch point of the bolter. He was taken to Middlesboro ARH where he received 6 stitches and x-rays showed no fractures.

Showing all 32EE stated that he was replacing the CLA on the conveyor pot of the miner. As the mechanic was raising the pan on the miner EE said that he stumbled and fell onto the pan. His left hand was pinned between the stop and the pan resulting in broken bones in the hand. EE was treated at the University of Tennessee Hospital and had pins and plates put in his hand.
Employee stated that he had his hand on the leveling arm of the roof bolt machine when he moved the head his finger was pinched. He received 6 stitches. He returned to work on the next shift.
Injury was using a drill in the installation of a mechanical belt guard. The drill bit hung and broke causing lacerations to the fingers and fractured his right index finger.
EE was roof bolting when the drill steel broke and struck him in the mouth. He was transported to the hospital and dentist where he recieved 5 stitches to the inside of his mouth. He also lost a tooth. EE returned to work on the next scheduled shift.
EE and other EE were attempting to open a gate valve on the heavy media suction line when EE's thumb was caught between the wheel on the valve and the valve itself. EE's thumb was dislocated and a mobility limiting brace was applied by the orthopeadic physician.EE has not lost any time from work.
Employee stated that as he was positioning the roof bolter the boom canopy struck a piece of rock causing it to fall striking him in the leg. The rock was appx. 15 inches x 15 inches x 4 inches thick. He continued to work until 5-10-08 when he was diagnosed with a blood clot.
Employee stated that he was roof bolting when the roof strap spun around and struck him in the nose. He was seen at the ER where he received 9 stitches to his nose. He returned to work on his next shift.
Employee had drilled a hole and stuck the roof bolt in the hole and was preparing to push the bolt up. The bolt didn't align with the wrench so he was going to bump the rotation and caught his glove around the roof bolt. He received 22 stitches to his left hand. He returned to work underground on his next scheduled shift.
Employee was operating the roof bolter when his left knee started hurting. He continued to work on regular duty without lost time until 1/17/07 when he underwent surgery for a meniscus tear.
As EE was trying to center bolt wrench in chuck, the wrench started to spin twisting right hand around wrench causing two fingers to be lacerated.
As employee was getting out of pinner, he put his hand on top of canopy. The pinner teeter-tottered causing employee's right middle finger to get caught between canopy and an installed roof bolt head. The finger was fractured.
Employee started to sit down to operate continuous miner when his right knee popped causing pain to the knee. Employee started working restricted duty the next day.
While installing a roof bolt, a piece of draw rock fell striking employee in the nose. His reaction to the falling material caused him to pull his head back, striking his head against the roof bolter canopy causing a neck strain. Employee started missing work on 7/25/2005.
EMPLOYEE WAS INSTALLING STRAPS ON A BROW WHEN ONE OF THE STRAPS TURNED STRIKING HIM IN THE FOREHEAD CASUING A LACERATION
A piece of coal came out of the miner conveyor and struck employee's right eye.
AS EE WAS TURNING A RIGHT BREAK, A PIECE OF DRAE ROCK FELL OUT BETWEEN BOLTS BREAKING HIS RIGHT LEG.
EE STOOD UP IN HIGH TOP AND COMPLAINED ABOUT PAIN IN HIS BACK. TREATED FOR BULGING DISK.
EE WAS TRAMMING MINER WHEN MINER COASTED INTO HIM CATCHING HIS KNEE BETWEEN MINER AND GOB MATERIAL.
EE WAS OPERATING THE MINER WHEN A ROCK ROLLED OFF THE RIB HITTING HIM IN THE SIDE AND FOOT.
AS EE WAS TRAMMING THE ROOF BOLTER TO NO B ENTRY HE RAN OVER A ROCK IN THE ROADWAY CAUSING HIS HEAD TO BE JAMMED INTO THE TOP INJURING HIS NECK & SHOULDER.
AS INJURED EE WAS REMOVING A CANOPY FROM THE ROOF BOLTER, A POST ON THE CANOPY FELL OVER STRIKING THE EE RIGHT THUMB. ABOUT 1/2 INCH OF THE THUMB WAS AMPUTATED.
AS INJURED EE WAS STRAIGHTENING ROOF BOLT AFTER PUTTING IT INTO THE DRILLED HOLE, HE BEGAN TO FELL PAIN IN HIS LOWER BACK AND COULD NOT STAND UP STRAIGHT.
AS EE WAS MOVING THE MINER A RIB ROLL OCCURRED INFLICTING A FRACTURE OF HIS LOWER RIGHT LEG.
WHILE BOLTING TOP, EMPLOYEE DROPPED A PIECE OF STEEL ON HIS LITTLE FINGER ON HIS LEFT HAND.
EE HAD HIS HAND ON THE DRILL BOOM WHEN A PIECE OF ROCK FELL STRIKING HIS LITTLE FINGER ON HIS RIGHT HAND.
EMPLOYEE WAS PUTTING UP BOLTS WHEN HE CAUGHT HIS3RD FINGER BETWEEN THE CHUCK AND THE BOLT HEAD.
EE WAS GRINDING WHEN THE GRINDER KICKED BACK OFF THE PIECE OF METAL AND CUTHIS LEFT KNEE.
EMPLOYEE WAS RUNNING A ROOF BOLTER WHEN A SMALL PIECE OF ROCK FELL CUTTING HIS RIGH ARM.
EMPLOYEE RELEASED TENSION ON BOLT AFTER RESIN HAD CURED. AS HE LOWERED BOLT WRENCH, RESIN CAMEOUT AROUND BOLT HEAD SPRAYING A SMALL AMOUNT INTO HIS EYES. MISSED WORK ON 3-20-00.
PIECE OF FINISHER STEEL WAS CAUGHT IN HOLE. INJURED TRIED TO GET IT OUT, THE PIECE OF STTEL FELL OUT STRIKING HIS HAND AND DRILL POT.
EE HAD RIGHT HAND ON PLATE WHEN HE STARTED ROTATION ON BOLT. PLATE CAUGHT GLOVE. PLATE JERKED OFF GLOVE TWISTING WRIST.
