EE was moving dirt with a D10 dozer when the dozer blade struck a rock and abruptly stopped the machine. The EE reported soreness to the neck and shoulder area and sought medical attention off site.

Miner (welder) was air arcing on a dragline bucket in the shop, after, when miner was repositioning the torch they bumped it. The hot stinger tip struck left arm and burned through welder coat, resulting in a third degree burn approx. 1" No Restrictions, No Lost Work Days, Treated with Silvadene Prescription.

A welder felt an irritation in his eye. He reported it to his supervisor and was taken to an optometrist for evaluation. A piece of metal was removed from his cornea.

Employee was welding, grinding. He was wearing sealed eye wear and a face shield when the wind picked up and debris got into his eye. He was seen at the Eye clinic and a rust ring was removed and antibiotics given.

Employee had been welding, cutting, grinding for the previous shift and the current shift. He felt an irritation in his eye. He had been wearing full PPE. He was taken to an optometrist and nothing was removed from his eye, however there was a small rust ring. He was given a prescription strength eye drop and returned to work full duty.

10/17/11 Dozer D11T 426 had trouble with the man lift it was latch in a up position and tagged out. On 10/19/11 The Mechanic & Operator worked on the man lift to repair a Bad Oil Reservoir The operator relied the latch pin and for the mechanic. The man lift dropped suddenly striking the mechanic in the left shoulder. Leaving a deep tissue bruise in the upper left arm.

7:30am supervisor was moving miner from the T-Dz to a RT-Dz. Miner couldn't stand or bend. Asked if he hit anything or fell, he didn't know what happen. Noted, during 1st-aid, he was using a back brace, He was checked by a MD said he was having spasms. I believed it was caused by a pre-existing condition. 08/30/2010 the miner claimed a LTA.

Two elec were cleaning collector rings in the dragline. Vacuum in the work area stirred up dust and they went to the shop to get respirators; they started coughing and could not get it under control. Both were taken to the clinic, tested and treated. Investigation showed the vacuum's motor stirred up the dust with just a filter on the vacuum; no bag corrections were made.

Welder was removing a wheel studs on a Coal Truck. After he was done welding a nut on one of the studs the welder raised his welding helmet. A piece of hot slag popped and flew behind his safety glasses where it lodged in his left eye. The piece had to be removed.

Welder was starting up air arcing after a short break. He turned the air back on. The air blew some debris around getting a small piece of metal behind his safety glasses and into his right eye. The metal piece was removed and he was back to work his next shift.

Employee, a Mechanic, was heating a wear sleeve on a D11R. He had a heating torch in one hand & was going to reposition a second torch with the other hand. He didn't get secure grip on the torch and slipped with it. He then tried to grab for it with his Rt. Hand and in the process received a 1st & 2nd Degree burn to his Rt. wrist, approx. 1.25 inches across.

Employee called me at Home at 10:00am Aug 12, 05 he said he had strained his mid back on 2nd shift Aug 11,05 about 7:00pm ripping coal. He told his supervisors. EE felt he should see his Chiropractor, call made. Appointment set Aug 15, 05. EE worked 2nd shift the 12th and continued his normal duties. I checked on him 4:00pm and asked how he was feeling he said fine but stiff.

Employee was using a 5/8" tugger wire rope connected to a 2" nylon sling that was chocked to the boom raising cable. The nylon sling received a cut from one of the drum clamp bolts causing the sling to tear in two. The tugger rope recoiled backwards with the socket end striking the employee on his hard hat.

EE was operating a motor grader blading subsoil when he struck a rock and jarred his back aggravating a pre-existing condition. He reported this to his supervisor and continued to work up until 8-18-04. At that time medical steps were taken to correct the problem.

EMPLOYEE WAS USING AN AIR IMPACT WRENCH TO TIGHTEN A NUT ON A HYDRAULIC CYLINDER. THE VIBRATION CAUSED THE CYLINDER TO MOVE AND IT FELL DOWN AND PINCHED THE TIP OF HIS THUMB.

EE WAS DRILLING A HOLE IN A POLE FOR A CROSS ARM. THE DRILL BIT CAME THROUGH THE POLE WHEN THE TIP OF THE BIT CAUGHT A GROUND WIRE THAT WAS STAPLED TO THE POLE. THE BIT STOPPED SUDDENLY AND TH E DRILL HANDLE SPUN AROUND AND HIT THE TOP OF HIS RIGHT HAND.

EMPLOYEE WAS PRE-RIPPING COAL WITH A RUBBER TIRE DOZER. AS HE BACKED UP THE RIGHT REAR WHEEL DROPPED INTO A HOLE ABOUT 1 1/2 FEET DEEP WHERE A CHUNK OF COAL HAD BEEN PULLED UP AND JARRED HIS B ACK. EMPLOYEE STATED HE COULD NOT SEE THE HOLE FROM THE MACHINE.

EMPLOYEE WAS HOLDING A PIECE OF METAL WITH HIS LEFT HAND TO BE WELDED IN PLACE AND THE WIRE FEED WELDING WIRE DID NOT ARCH AND THE WIRE DEFLECTED THROUGH HIS LEATHER GLOVE INTO HIS THUMB. HE W ENT TO THE DOCTOR AFTER WORK TO BE CHECKED.
