EMPLOYEE HAD OPENED MAIN VALVE ON ANHYDROUS AMMONIA TANK. END OF HOSE WAS IN MUD. PRESSURE BUILT UP IN HOSE AND BLEW OFF SECONDARY VALVE ON HOSE CAUSING ANHYDROUS TO LEAK INTO THE AIR.
