Employee states that he had pushed to a bench and was in the process of turning to go back up the slope when a rock dislodged causing the tractor to slide over the bank onto the machines right side. This resulted in the tractor overturning approximately three times before coming to rest in an upright position. Received 9 sutures.

Showing all 23Employee stated he and another employee were using air chisels to remove a bearing race from a bearing assembly for the hoist drum on the 495 shovel. In doing so, the chisel tool had a piece of metal chip off and struck the employee in the left leg just below the knee.
A contract mechanic received a laceration to the side of his head while attempting to run the boom on the service truck out. Mechanic toggled the boom swing button and he meant to press the boom extension button. Mechanic was taken to CAMC General ER where he received 4 sutures to close the wound. He returned to work on 12/10/2013.
Foreign object entered eye while chipping weld slag or grinding weld.
Employee was using a grinder with a cut off disc polishing a piece of metal when the disc fragmented. Fragments struck the left thumb requiring three sutures and struck the left forearm requiring five sutures
After shift, employee discovered metal particle in his right eye. He had been using an air impact wrench while wearing safety glasses with side shields. He did not feel particle enter the eye. He reported to work next day, 7th. An attempt was made to wash particle out without success and he was sent to an eye specialist.
EE WAS CUTTING A TREE WHEN THE TREE BEGAN TO SPLIT. EE TRIED TO ESCAPE TREE BY RUNNING UP THE HILL. HE SLIPPED AND FELL CAUSING HIMSELF TO BE UNDER THE TREE AS IT CAME DOWN BREAKING HIS LEG.
Building rock core for valley fill. Dozer pushing rock to excavator that had catch bench in front. Rock rolled from dozer. Excavator caught rock but rock sheared and half struck excavator. Rock broke windshied and glass struck operator's right shin causing laceration that required nine sutures to close. Employee returned to work next available shift.
During Part 50 Audit MSHA has determined that this injury should have been reported as the employee missed no days but worked two short shifts. Employee driving rock truck and being loaded by shovel was jolted forward in drivers seat when large rock struck the bed. Employee complained of neck pain but did return on his next work shift in his regular job.
USING 9"GRINDER BETWEEN 2 EARS ON A DOZER BLADE THE GRINDER GRABBED & BECAME BINDED BETWEEN 2 EARS & PINNED LEFT THUMB BEHIND THE GRINDER & METAL. EE HAD HIS WELDING GLOVES ON & THAT IS WHAT SAVED HIS HAND & THUMB FROM BEING MORE INJURED.
EE WAS USING STAINLESS WIRE WHILE WELDING ON MANGANESE LIP ON THE 4100 SHOVEL BUCKET. WEATHER CONDITIONS WERE RAINY AND WINDY. HE HAD AFFIXED A TARP OVERHEAD. WIND MAY HAVE CHANGED DIRECTIONS TO WHERE HE GOT A GOOD WHIFF OF SMOKE. HE FELT LIGHT HEADED AND SOME DISCOMFORT BUT RETURNED TO WORK. FOLLOWING DAY HE CALLED IN SICK BECAUSE OF SMOKE INHALATION.
EE WAS USING AN ACETYLENE-CUTTING TORCH TO CUT THE WELD OFF OF THE RIGHT SIDE CORNER BIT BOLTS WHEN A PIECE OF HOT SLAG BOUNCED OFF A GUSSET ANDENTERED HIS RIGHT EAR. EE CONT. TO WORK WITHOUT SEEKING ANY MEDICAL ATTN. AS TIME PASSED, HE SUFFERED DRAINAGE FROM THE EAR THAT WOULD NOT CLEARUP.HE SOUGHT MEDICAL TREATMENT AND DISCOVERED SMALL HOLE IN EARDRUM. SURGERY ON MARCH 26TH
EE WAS GRINDING WELD ON THE SHOVEL BOOM. CONDITIONS WERE WINDY. APPARENTLY THE WIND BLEW A METALPARTICLE INTO EE'S LT. EYE. THE EE WAS WEARING HIS HOOD WITH SAFETY GLASSES. THE EE FELT IRRITAT ION IN THE EYE BUT FIRST THOUGHT HE MAY HAVE SUFFERED SOME WELDING FLASH BURN. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 2 DAYS LATER WHEN THE EYE BECAME MORE IRRITATED & METAL DISCOVERED.
EE WAS GRINDING WITH AN OPERATED GRINDER ON A NOSE CONE FOR A JACK WHEN THE GRINDING DISC SPLIT IN HALF STRIKING HIM IN THE FACE AT THE BREAK OF HIS NOSE CAUSING A SERIOUS CUT TO THE NOSE WHICH REQUIRED STITCHES.
EE WAS WELDING ON THE DRILL FRAME. WITH THE WELDING HOOD ON LOOKING THROUGH THE CLEAR LENS HE WAS CHIPPING SLAG WHEN A GUST OF WIND BLEW A METALPARTICLE IN BEHIND THE WELDING HOOD AND INTO HIS LT. EYE. HIS EYE WAS WASHED OUT AND HE CONTINUEDTO WORK THE REMAINDER OF SHIFT. THE EYE BECAME MORE IRRITATED. HE WENT TO THE CLINIC AND A PIECE OF METAL WAS FOUND EMBEDDED IN HIS EYE.
EE WAS GRADING HAULROAD UNDER VERY MUDDY CONDITIONS. AS HE WAS GRADING, THE BLADE OF THE GRADER STRUCK A LARGE ROCK. THE SEAT BELT RESTRAINED HIM IN THE SEAT BUT HIS UPPER BODY WAS THROWN FORW ARD IN A WHIPLASH ACTION RESULTING IN A SPRAIN TO HIS LOWER BACK. THIS WAS HIS LAST SHIFT OF THE WORK WEEK AND HE ASKED TO LEAVE EARLY DUE TO DISCOMFORT IN HIS BACK. ON HIS 1ST SCHEDULED DAY
STANDING NEAR WELDER - FLASH BURN (R) EYE.
AFTER COMPLETING REPAIR OF THE 2570 DRAGLINE BOOM FROM THE MAN-BASKET, EE ASKED TO BE LOWERED BACK TO THE GROUND. THE MAN-BASKET OPERATOR MISTAKENLY GRABBED & PULLED THE SWING LEVER AS OPPOSED TO THE RETRACT LEVER. EE WAS HOLDING THE TOP OF THE MAN-BASKET WITH BOTH HANDS. WHEN IT SWUNG, IT PINCHED HIS LEFT INDEX FINGER AGAINST A PIPE ON THE BOOM & THE MAN-BASKET, LACERATING IT.
EE HAD A PIECE OF FLAT BAR STEEL WHICH MEASURED 3/4"X6"X13' LEANING ON THE BUMPER OF HIS SERVICE TRUCK. HE CUT A 29 INCH PIECE OFF OF THE UPPER END OF THE STEEL. WHEN THE CUT WAS COMPLETED, TH E REMAINDER OF THE STEEL TOE PROTECTION OF HIS BOOT. THE IMPACT FRACTURED A SMALL BONE IN THE TOP OF HIS FOOT.
EE WORKS ON SECOND SHIFT AMINTENANCE AS A WELDER. HE WORKED HIS REGULAR SHIFT ON 1-16-01. AS HE DROVE HOME AFTER THE SHIFT, HE FELT DISCOMFORT IN HIS RIGHT EYE. HE WORKED HIS REGULAR SHIFT ON 1-17-01. THE EY HAD BECOME RED AND WAS MORE PAINFUL. ONE HIS WAY HOME AFTER HIS SHIFT, HE STOPPED AT THE EMERGENCY ROOM OF LOCAL HSPITAL. THE EYE WAS EXAMINED AND A SMALL METAL FRAGMENT WAS NO
CUTTING TREES
EE WAS OPERATING THE BACKHOE. HE WAS ATTEMPTING TO MOVE A LARGE ROCK WHEN THE BUCKET OF THE MACHINE SLIPPED OFF THE ROCK. THIS CAUSED THE MACHINE TO ROCK SUDDENLY. HE WAS BOUNCED BACK INTO THE SEAT. THE RESULT WAS A CERVICAL SPRAIN.
EE WAS USING A GRINDER TO REMOVE BOLTS FROM THE CATWALK ON THE MAST. HE WAS WEARING SAFETY GLASSES. WINDY CONDITIONS. WHILE CUTTING, A METAL PARTICLE GOT IN BEHIND THE GLASSES AND INTO HIS RIG HT EYE. DID NOT NOTICE AT THAT TIME. LATER ON, FELT EYE GETTING IRRITATED. FELLOW EE SAW IT IN THE CORNER OF THE EYE. WENT TO ER WHERE IT WAS REMOVED.
