EMPLOYEE INSTALLED ROOFBOLT ALONG RIB HE THEN LET THE ATRS DOWN IN ORDER TO ENABLE HIM TO SWING HIS BOOM TOWARDS THE CENTER OF THE PLACE TO INSTALL ANOTHER ROOF BOLT.INSTEAD OF WALKING THE TWO STEPS BACK TO ACTIVATE HIS ATRS THE EMPLOYEE STRETCHED TO REACH THE LEVERS.HE THEN LET THE ATRS(AUTOMATIC TEMPORARY ROOF SUPPORT) DOWN AND IT CAME IN CONTACT WITH HIS EXTENDED RIGHT LEG.

WHILE OPERATING THE OFF SIDE OF THE DUAL BOOM SIMMONS RARD ROOF BOLTER A LEAK THE SIZE OF A PIN HOLE DEVELOPED IN THE HYDRAULIC HOSE GOING TO THE ROTATION. THE STREAM OF OIL STRUCK THE EE IN T HE LEFT EYE EE WAS IMEDIATLY TAKEN TO THE HOSPTIAL EMERGENCY ROOM THE ATTENDING PHY FORWARD NO DAMAGE OR FOREIGN MATTER.
