EE was moving the roof bolter when he stepped and his arm hit the slide lever, and the slide came out and mashed his middle finger on his left hand. Received 15 Stitches on his finger. No lost time.
He was installing roof bolts and pulled a muscle in his chest.

He was roof bolting when the drill steel got stuck. He tried to get it out when he grabbed the wrong lever and it pulled the canopy over on his right hand. It fractured his right hand/wrist.
Pinning top. The pin wrench came off and hit him in the mouth. He received 2 stitches, bottom lip. No lost time.
EE was roof bolting with another EE, when the drill steel stopped up with mud. While attempting to clean, the drill came loose and hit EE on the forehead, causing a small cut. He received two stitches. No lost time.
EE WAS TRAMMING ROOF BOLTER. WHILE WATCHING CABLE TAKE UP, HE HIT HIS HEAD ON RIB (STRAINED NECK).
EMPLOYEE WAS BOLTING ROOF WHEN DRILL STEEL BROKE. IT CAME BACK HITTING HIM IN THE RIGHT ARM, BREAKING HIS FOREARM.
THE EMPLOYEE WAS INSTALLING 5' ROOF BOLT WHEN HE FELT PAIN IN LOWER RIGHT SIDE.
THE EE WAS ROOF BOLTING WHEN THE BOLTS WORKED OUT OF THE VALVE CHEST AND THE HOLES GOT CAUGHT AND IT MADE THE VALVE CHEST COME UP AND HAND BETWEEN CHEST AND FRAME CAUSING CONTUSION TO TOP OF L EFT HAND.
THE EE WAS IN THE PROCESS OF ROOF BOLTING WHEN A PIECE OF DRAW ROCK FELL (1 1/2'LONG, 1 1/2'WIDEX 3"THICK). THE EE HAD HIS HAND ON PINNER & WAS GOING TO PUSH SELF AWAY WHEN THE ROCK FELL HITTI NG HIM ON LT. HAND, LACERATING MIDDLE FINGER. 20SUTURES WERE REQUIRED.
THE EMPLOYEE WAS OPERATING REMOTE CONTROL MINER TURNING RIGHT BREAK WHEN A PIECE OF DRAW ROCK FELL BETWEEN BOLTS & HIT EMPLOYEE ON THE HEAD.
THE EE WAS GETTING READY TO SET MINER OVER IN #6 HEADING WHEN A PIECE OF DRAW ROCK FELL IN BETWEEN BOLTS CAUSING LACERATION TO BACK OF HEAD.
EE WAS OPERATING ROOF BOLTER PUTTING UP BOLT, HE DROPPED POD AND PUSHED BOLT UP, DROPPED POD DOWN AND PUT WRENCH ON. EE SHOVED POD UP AND SAVED IT AGAINST POD SMASHING LEFT THUMB.
THE EE WAS OPERATING MINER WHEN A PIECE OF DRAW ROCK FELL, STRIKING HIM ON THE LEFT ARM & LEFT FOOT & CAUSING ABRASION TO FOREHEAD.
EE HAD PUT FINISHER IN HOLE WITH LEFT HAND AND SWUNG PAD AROUND AND SMASHED LEFT HAND AGAINST STEEL REQUIRING 7 SUTURES.
