Employee was using an impact gun & socket. When a pin came out of the socket, it struck the palm of employee's Left hand, resulting in a puncture wound that required suturing.
On January 18th, 2010, the accident victim reported to his supervisor that on January 16th, 2010, he had become injured when he was backing up his dozer and it struck the ground forcefully, injuring his neck and upper back area (between his shoulder blades). The injured employee then began missing work as a result on January 19th, 2010.
Employee dropped rock duster on his right hand, crushing & lacerating his middle finger.
A bulldozer became lodged on a tree stump, causing it to overturn when operator tried to remove it.
Employee was in the operator's deck backing the Fletcher Roof Bolter out of the #8 heading, when he got his Left ring finger caught between a trailing cable and the frame of the roof bolter, causing a laceration that required stitches.
The employee was bolting the #7 break when his drill steel became lodged in a hole, then came loose unexpectedly, striking the ring finger of his left hand. The incident resulted in a lacertaion that required stitches.
The injured employee was crawling toward the roof bolt machine when his left foot slipped off the coal and became caught underneath the wheel of the machine.
THE EMPLOYEE WAS POSITIONED OUTBY THE 2ND ROW OF BOLTS & WAS BOLTING THE TOP IN THE NO.3 LEFT CROSS-CUT. WHEN THE ATRS WAS LOWERED, A PIECE OF ROOF ROCK FELL & STRUCK HIS FOOT.
A ROCK FELL FROM BETWEEN THE RIB AND BOLT AND ONTO THE EMPLOYEE'S FOOT AND ANKLE.
INJURED EE TOLD THE WITNESS THAT HE WAS GOING TO MOVE THE CABLE CROSSER. HE DID NOT HEAR HIM AND ACCIDENTALLY RAN OVER HIS RIGHT HAND WITH A ROOF BOLTER WHEEL.
ADVERSE ROOF. * EMPLOYEE HIT WITH FALLING ROCK WHILE ROOF BOLTING *
EMPLOYEE WAS INSTALLING A BOLT WHEN HIS THUMB ON HIS RT.HAND SLIPPED UNDER THE BOLTHEAD AND WAS PINCHED BETWEEN THE DRILL POT & BOLTHEAD CAUSING A FRACTURE TO HIS RT.THUMB & REQUIRING STITCHES.
EE WAS PUSHING A BENT 4' ROOF BOLT UP IN A DRILLED HOLE AND ACCIDENTALLY HIT THE DRILL ROTATION CAUSING THE ROOF BOLT TO ROTATE AROUND AND STRIKE EE ON BACK OF LEFT HAND CAUSING A FRACTURE.
WHILE INSTALLING ROOF BOLTS, DRAW ROCK FELL CATCHING THE OPERATOR'S RIGHT MIDDLE FINGER AGAINST THE DRILL POT ON THE MACHINE (REQUIRED STITCHES).
AS THE EE BEGAN TO DRILL, A SMALL PIECE OF DRAWROCK FELL STRIKING HIM ON THE RIGHT HAND. HE WAS TAKEN TO THE HOSPITAL AND RECEIVED STITCHES TO HIS RIGHT HAND,.
EE WAS EXHANGING BOLTS WHEN A PIECE OF DRAW ROCK FELL OUT FROM BETWEEN ROOF BOLTS AND STRUCK EE ON TOP OF HIS HARD HAT.
