G M Mining Inc · Struck by falling object

HE WAS INSTALLING ROOF BOLTS IN #6 RIGHT BREAK. HE HAD PUT UP THE FIRST RIB BOLT AND WAS PREPARING TO PUT UP THE SECOND BOLT WHEN A PIECE OF DRAW ROCK BROKE OUT BETWEEN THE ROW OF BOLTS HE WAS INSTALLING AND THE ROW OUTBY. THE ROCK FELL ON THE TRAY OF THE BOLTER AND SLID OFF ON THE LEFT FOOT OF THE ROOF BOLTER OPERATOR.
