EE was working at the Goals mine location cutting brush when a stick struck EE in the eye. EE claims EE was wearing a hard hat with face shield. EE's supervisor transported EE to Beckley General hospital in company vehicle. Beckley General transported EE to Ruby Memorial hospital via ambulance.

Employee was using an electric drill and the handle slipped causing the employee to cut his hand on the drill bit. Causing the employee to receive 5 sutures in his hand.

EMPLOYEE STATED THAT WHILE PUSHING CLEAN COLE AND REFUSE HE BECAME ILL, IN THAT HIS FACE BURNS, CHEST FEELS HEAVY AND THROAT BECOMES SORE. SAYS SYMPTOMS HAVE BEEN PREVALENT ABOUT 3 WEEKS. RKERS COMP FORM INDICATED ILLNESS WAS ATTRIBUTEDTO OCCUPATIONAL FUME EXPOSURE.
