WHILE PUSHING THE BOLT INTO THE H OLE WITH THE HEAD OF THE ROOF BOLTER, HE INADVERTENTLY PLACED HIS LEFT HAND BETWEEN THE ROOF BOLT PLATE AND THE ROOF, MASHING HIS HAND BETWEEN THE PLATE AND ROOF.
