IE decided to switch to a welding hood but couldn't see the lines on the weld setting so IE switched to grind setting. IE woke us the next morning with eyes burning from flash burn. IE received prescription eye ointment.

Showing 50 of 52IE was lowering the classifying cyclone lid on the cyclone on the 6th floor of the prep plant. IE pulled the chain tail of the hoist back and it whipped IE on the lip. IE received 2 stitches.
IE was setting a 9ft bolt and the long wrench broken at the chuck, the head of the wrench cut IE left arm. The IE laceration needed 12 stitches.
IE was roof bolting when EE grabbed the wrong lever closing the jaws around EE's right hand causing a fracture to fourth metacarpal shaft fracture.
While roof bolting the IE was attempting to move the roof bolter cable out of the way while IE was installing a bacon strip. When the IE stepped back down onto the platform IE snagged the mast feed cylinder with IE's clothing causing the mast to move down pinching IE's right arm between the mast and roof bolt plate tray.
IE was grading out of the tail-gate when a roof bolt was kicked out by the lump breaker striking the IE above the left eye causing a laceration that required stitches to close the wound. It also caused a tooth to be chipped.
IE was roof bolting in second right X-Cut 1-2 when they inadvertently closed the jaws on their left hand causing a laceration on the left middle finger that required stiches to close the wound.
I.E was trying to straighten frame work on the tailgate drive with a crib block between the anti slew bracket and tailgate drive frame. I.E was holding the crib block while another employee was operating the shields pulling the pan-line back. The crib block popped out striking I.E in the face knocking I.E unconscious and causing a fracture to zygomatic bone.
While roof bolting employee got right ring finger caught in the jaws amputating the tip of employee's finger.
While roof bolting the employee's drill steels got stuck in the roof mesh. The roof bolting partner set angle bolt causing the roof mesh to move freeing the steels from the mesh. The steels came out of the roof striking the employee's right pinky finger that was on top of the jaws causing a wound that required stitches. Right pinky finger also has a small fracture.
While roof bolting employee got left thumb caught between the roof bolt and the back of the jaws causing a laceration that required stiches to close the wound.
Employee was tightening flinger plate on chuck, raised his drill steels and closed the jaws to hold drill steel up out of the way when his partner set his bolt the drill steels fell about 2' striking employee's middle finger on his left hand, causing 2 small lacerations that required stitches and a fracture to the bone.
The employee was setting a shield on the new long wall face. Before the shield could be set, the side shield needed to be pulled in. EE was kneeling down and had his hand in wrong position. Another employee closed the side shield after giving warning and caught EE's hand between the shield and side shield.
The employee was installing a roof bolt underground and caught his little finger between the clamp and the bolt. ***MSHA reportable injury***
The EE was holding the multi-cable hook out of the way, while the roof bolter was trammed out of the entry. The roof bolter slid in the mud and swung over and smashed his left ring finger.
The employee was using a pipe wrench to remove a stuck drill steel in the chuck of the roof bolter. He hit the rotation lever by mistake and the wrench spun around and struck him in the face.
The employee was bolting on the right side, the bolt was at an angle and he was using a wrench to hold it. The bolt spun and caused his finger to be smashed between the jaws and wrench.
The EE was installing a 4' roof bolt. He attempted to connect the wrench with the drill steel. His hand slipped off the joy stick, causing him to activate the steel clamp, which closed on his right thumb and middle finger.
The employee was standing on a shield that was activated. His foot was caught between the shielf toe and the panline.
The employee was roof bolting and pulling the drill steel out of the roof, it slipped and smashed his finger against the bolter. ***MSHA restricted duty 02-12-07.
Employee was using a grinder to clean up a torch cut on a conveyor bent. The grinder kicked back and struck the employee's cheek. Employee was wearing proper safety equipment (shield, safety glasses, & gloves).
While employee was operating the shuttle car, small rocks were coming off the tail of the continuous miner. The employee leaned forward when a bigger rock bounced off the cab of the shuttle car and hit him in the upper back and ribs. The employee requested to see a doctor on 6/13/06 and was diagnosed with a rib fracture. **MSHA reportable injury**
THE EMPLOYEE HAD BEEN ROOF BOLTING AND COMPLAINED THAT HE WAS EXPERIENCING PAIN IN HIS RIGHT SHOULDER. HE WENT AND SAW A DR ON 8/11/05 AND WAS GIVEN A LIGHT DUTY RELEASE AND RTW 8/12/05
The employee was rock dusting underground when the dust line surged and threw him into the rib. He fell and landed on a pile of coal or his SCSR.
WHILE THE EMPLOYEE WAS ATTEMPTING TO UNHOOK A WINCH HOOK, OFF OF THE WAGNER, THE OPERATOR HIT THE WINCH LEVER WITHOUT GETTING THE SIGNAL FROM HIM AND HIS FINGER WAS SMASHED. 1 STITCH WAS REQUIRED TO CLOSE THE LACERATION

THE EMPLOYEE AND CREW WERE EXTRACTING AN OVERCAST WALL. THE OPERATOR HIT THE WALL WITH A WAGNER BUCKET. THE FRAMEWORK THAT WAS BUILT INTO THE WALL POPPED OUT AND FELL TOWARD THE EMPLOYEE. HE TURNED TO RUN AND WAS HIT BY THE FRAME FROM BEHIND. IT DROVE HIM TO THE GROUND. LOST TIME ACCIDENT; FIRST DAY OUT 6/17/05.
THE EMPLOYEE WAS JACKLEGGING THE RIBS IN B-MAINS. HE WAS STANDING ON THE HANDLE OF THE JACKLEG AND WAS DRILLING AT A HEIGHT OF ABOUT 12 FEET WHEN THE PIG TOE SPUN AND THREW THE EMPLOYEE TO THE GROUND. HE LANDED ON HIS BACK ON A ROCK. RESTRICTED DUTY 6/15/05.
THREE EMPLOYEES WERE WORKING TO PULL THE TAILGATE DRIVE BACK. THE GOB PLATE WAS ATTACHED TO THE SHIELD WITH CHAINS. AS THEY STARTED TO PULL, ONE OF THE CHAINS BROKE AND STRUCK EE IN THE FACE, CAUSING A FRACTURE TO HIS CHEEK AND A LACERATION WHICH REQUIRED STITCHES. REPORTABLE INJURY.
The employee was roof bolting when he went to drop the mast and pulled the wrong lever. He set the jaws on his right hand causing a smashing injury to that hand. He was placed on restricted duty 8/24/04.
The employee was jack legging the ribs when a large chunk of coal fell out of the rib and struck him on the head and shoulder. The employee did not have any broken bones or internal injuries. *First day out was 8/13/04.
THE EE WAS REMOVING ROCK AND COAL CHUNKS FROM THE BRETBY TROUGH WHEN THE FACE BLEW OUT AND A BIG CHUNK OF COAL CAUGHT HIS HAND BETWEEN THE PANLINE, SMASHING HIS RIGHT HAND AND CAUSING A FRACTURE TO HIS RIGHT MIDDLE KNUCKLE. FIRST DAY OUT: 6/15/04.

WHEN IT CAME LOOSE IT HIT HIS RIGHT HAND, THE WEBBED PART IN BETWEEN THE THUMB AND FINGER, AGAINST THE DRILL HEAD CAUSING A LACERATION. SUTURES WERE REQUIRED TO CLOSE THE LACERATION. THE EE WAS ROCK DUSTING. THE DUST LINE HE WAS USING HAD A HOLE IN IT. THE DUST LINE SURGED, CAUSING THE ROCK DUST TO SQUIRT OUT OF THE HOLE INTO HIS RIGHT EYE UNDER HIS SAFETY GLASSES. THE EYE WAS WASHED OUT AND THE EE WENT TO SEE THE DOCTOR WHO PRESCRIBED EYE OINTMENT TO HELP THE HEALING PROCESS. REPORTABLE INJURY.
THE EMPLOYEE WAS INSTALLING RAILROAD TIES WHEN THE TIE CAME LOOSE IN THE CLAMP JAWS. THE TIE TIPPED UP AND SMASHED HIS FINGER BETWEEN THE TIE AND THE BOOM OF THE INSTALLER. THE EMPLOYEE FRACTU RED THE TIP OF HIS RIGHT 5TH FINGER. HE RETURNED TO WORK 09/18/03.
THE EE WAS BOLTING THE LONGWALL FACE WHEN THE BIT LOST IT'S BITE AND SLID ACROSS THE TOP AND THE DRILL RAISED UP SMASHING HIS FINGER BETWEEN THE SHIELD AND THE DRILL STEEL. RESTRICTED DUTY ON 08-11-03
THE EMPLOYEE WAS SINKING AN ANCHOR BOLT INTO THE FLOOR WITH A JACK HAMMER. THE HAMMER SLIPPED OFF THE BOLT CAUSING THE EMPLOYEE TO OVEREXTEND HIS RIGHT ARM/SHOULDER.
THE EE WAS TRYING TO STRAIGHTEN OUT A 16 FOOT CALBE BOLT WHEN HE SLIPPED AND TWISTED HIS BACK.
EMPLOYEE WAS GRINDING UNDERNEATH THE SCRUBBER & BETWEEN THE FRAME OF THE CONTINUOUS MINER. THE GRINDER KICKED BACK, STRIKING HIS LEFT CHEEK AND FOREHEAD. HE REQUIRED STITCHES & RETURNED 1/14.
THE EMPLOYEE WAS OPERATING THE CONT MINER BY REMOTE CONTROL. HE FILLED THE SHUTTLE CAR TO CAPACITY AND A CHUNK OF COAL FELL OFF AND STRUCK HIS ANKLE. THE EE WENT IN AND SAW THE DOCTOR.
WHILE MINING IN THE COAL PILE BEHIND THE MINER IN ORDER TO CLEAN UP THE PILE, TRAMMING OVER THE LOOSE COAL THE MINER SLID TOWARD AND AGAINST THE OPERATOR, PINCHING HIM BETWEEN THE MACHINE AND THE COAL RIB. HE WAS TRANSPORTED TO THE HOSPITAL WHERE TREATMENT BEGAN FOR HIS INJURY.
EE WAS ROOF BOLTING AND WAS IN THE PROCESS OF DRILLING A HOLE. WHEN HE WENT TO REMOVE HIS STEEL THE STEEL CAME OUT OF THE DRILL CHUCK AND PINNED HIS LEFT PINKY FINGER BETWEEN THE JAWS AND THE STEEL, WHICH RESULTED IN A LACERATION TO THE FINGER. SUTURES WERE REQUIRED TO CLOSE THE LACERATION. EE RESUMED HIS REGULAR DUTIES 2/27/02.
THE EE WAS ROOF BOLTING. HE WENT TO REMOVE THE DRILL STEEL FROM THE HOLE, AS HE PULLED THE STEEL IT CAME LOOSE HITTING HIS LT. FINGERS. THIS CAUSED A FRACTURE TO HIS 3RD AND 4TH LT. FINGERS SUTURES WERE ALSO REQUIRED TO CLOSE THE LACERATION TO BOTH FINGERS. THE EE WAS RELEASED TO RESTRICTED DUTY. 1ST DAY RESTRICTED DUTY: 2-16-02.
THE EE WAS OPERATING THE SHEARER IN THE LONGWALLSECTION. HE WAS CHECKING THE CUTTING LEVEL OF THE DRUM WHEN THE FACE BLEW OUT AND A PIECE OF COAL STRUCK HIS BOTTOM LIP. HE RECEIVED 4 STITCH ES TO CLOSE THE LACERATION. RETURNED TO WORK HISNEXT SCHEDULED SHIFT ON 1-28-02.
THE EMPLOYEE WAS ROOF BOLTING. WHILE SETTING THEBOLT, IT STARTED TO BEND. HE THEN HIT THE WRONG CONTROL; INSTEAD OF LETTING IT DOWN HE HIT THE UP CONTROL & FRACTURED HIS RIGHT RING FINGERTIP.
THE EMPLOYEE WAS USING A 7" METAL GRINDER TO CUTLOOSE TACK WELDS ON A CHUTE PIECE. THE EMPLOYEE GROUND THE WELD OFF AND AS THE WELD CAME APART THE GRINDER HUNG UP FOR A SECOND AND THEN JERKED AROUND AND CUT THE EMPLOYEE'S LEFT WRIST. THE EMPLOYEE RECEIVED 7 STITCHES TO CLOSE THE LACER-ATION AND RETURNED TO WORK THE NEXT DAY REGULAR DUTY.
THE EE WAS CABLE BOLTING AND A 5 FOOT STEEL STUCK IN THE CHUCK. WHEN HE WAS GETTING IT OUT THE 11 FOOT OF STEEL CAME OUT OF THE HOLE AND PINCHED HIS FINGER BETWEEN THE TWO STEELS. THE EE HAS A FRACTURED FINGER. A SPLINT WAS PLACED ON THE FINGER AND THE EE RETURNED TO WORK ON 10-15-01 HIS NEXT SCHEDULED WORK DAY.
THE EMPLOYEE WAS ROOF BOLTING AND GOT SOMETHING IN HIS RIGHT EYE. THE NEXT MORNING IT CONTINUED TO BOTHER HIM AND HE WENT TO THE DOCTOR. A SMALL PIECE OF MATERIAL WAS REMOVED FROM HIS EYE. HE HAS SEEN THE DOCTOR AGAIN 5-3-01 AND WAS RELEASED TO WORK WITHOUT RESTRICTIONS.
THE EMPLOYEE WAS HELPING BOLT THE RIBS WITH MESH. HE WAS INSTALLING RESIN INTO THE DRILL HOLE. THE RIB ROLLED OUT AND STRUCK THE EE ON HIS LEFT SIDE AND HIS LEFT FOOT. THE EE RECEIVED A SPRAIN ED LEFT FOOT AND A SMALL CONTUSION TO HIS LEFT EAR. **RESTRICTED DUTY ON 2/19/01.
THE CONTINUOUS MINER OPERATOR AND THE EMPLOYEE WERE BACKING THE CM OUTOF THE #1 ENTRY. THE EMPLOYEE WAS PICKING UP THE ELECTRICAL CABLE AND PLACING IT ON THE BOOM OF THE CM. THE CMO STOPPED TH E CM, AT WHICH TIME THE CM SLID FORWARD AND TO THE RIGHT HITTING THE EMPLOYEE IN THE RIGHT FRONT SIDE OF THE LOWER ABDOMEN AND PUSHED HIM INTO THE RIB. EMPLOYEE SENT FOR MEDICAL TREATMENT.
