EE was helping replace a worn out screen, when installing the new screen, the pan raised up and smashed his finger between pan and I-Beam.

While roof bolting, his pin plate got stuck on the ATRS, he reached up to clear it and the plate broke free pinching his right middle finger. ***stitches***

While installing roof bolts and hog wire to the mine roof, ee used his left hand to hold up the hog wire, the hog wire got wrapped around rotating steel, pulling his left arm around the steel.(causing the amputation of his left arm just below the elbow)

While installing a roof bolt, gob fell out from between the pins, it hit the canopy first and rolled under striking him in the left calf. ***stitches***

Was standing on opposite side of bolter, putting bits on stringer, when operator trammed bolter backward, running over top of left foot.

While cutting wood with a circular saw, he hit a knot and the saw jump back cutting his right thumb. *** He received 7 stitches ***

While putting up a corner pin, somehow hit too much up pressure bending the drill steel and when rotated hit him in the left shoulder, causing stitches to be needed.

While installing chain hangers at the face of #3 Unit's beltline, the line curtain became wrapped in the drill steel and pulled his right hand into curtain. ***He began missing on 12-4-2012.***

While backing the bolter out of #9 Entry, the bolter began to sink in the bottoms and slide towards the rib. After hitting a big rock, the bolter jumped, throwing the operators foot out of the cab and smashing it between the rib and the bolter. He received a fracture in his left foot. ***He began missing on 06/04/2012.***

He was using an electric grinder on a belt drive, a small piece of metal got in his left eye. ( he was wearing safety glasses at the time) The piece of metal was removed.

Operator was using the boom of the bolter to dig out the mine floor to gain more height. He did not have the foot jack set so when he raised boom up the canopy dropped striking him on the head, jamming neck. Doctors thought it could possibly be a protruding disc and is put off work as of 8-4-11 during physical therapy.

The operator was trying to align chuck with pinner steel, he then started the rotation and the steel popped out and struck hand between index finger and thumb.

The operator was holding onto drill steel while lining it up with the chuck. When the chuck came into contact with the steel, the steel kicked back striking the operators left hand between the thumb and index finger. Resulting in several broken bones.

He was installing roof bolt with a pin board attached, when pushing bolt to top, he caught his right index finger between pin board and top. ( Needing stitches on end of right index finger.)

While roof bolting, his glove got caught on pinner steel and jerked shoulder. ** started missing on 09/28/09**

He was using a grinder on golf cart when a piece of metal went under his glasses into his eye.

HE WAS REMOVING MINER CABLE STRAP FROM THE TAIL HOOK ON THE MINER, WHEN HE WAS PINNED BETWEEN THE COAL RIB AND THE TAIL OF THE MINER.

WHILE PUTTING UP BRAWL BARS, HE PLACE HAND ON PINNER STEEL WHILE ROTATING AND PULLED HAND AROUND STEEL.

HE WAS BOLTING UP DRIVE SHAFT TO TRANSMISSION, WHEN THE JEEP MOVED, CAUSING IT TO ROLLED OFF CRIBS.

Employee was roof bolting when a connector on the drill steel broke, flew up, and struck him on the right eyelid. Employee stated he was wearing safety glasses, but the piece got through somehow. He received a laceration that required 5 stitches.

Injured man was in the process of bending a 7 foot roof bolt for installation. He placed his foot on the front of the bolter drill head for stability. As he went to bend the bolt his foot slipped and he felt pain in his lower back.

HE WAS STARTING HIS STEEL, WHILE IT WHIPPED AROUND STRIKING HIM ON THE HAND.

EMPLOYEE WAS ADDING SECTION TO PINNER STEEL, WHEN THE BOTTOM HALF OF STEEL CAME LOOSE FROM TOP SECTION, HITTING HIS THUMB.

EMPLOYEE WAS BENDING OVER TO PICK UP THE CABLE WHEN THE MINER COASTED ON AN INCLINE, THE DRUM OF THE MINER HIT HIM IN THE SHOULDER PUSHING HIM TO THE GROUND.

After employee was finished putting up pin, his partner told him that a rock was on the canopy. As employee lowered the conopy the rock slid off, striking him in the lower back and hip area. The rock was approximately 1' x 2' x 1/2" in size.

HE WAS MOVING MINER, WHEN THE CABLE CAUGHT HIS FOOT AND TWISTED HIM AT THE KNEE. ** STARTED MISSING WORK **8-03-05**

EE WAS LINING UP DRILL STEEL. STEEL POPPED LOOSE, JAMMING INTO EE'S HAND BETWEEN THUMB AND FOREFINGER.

EE WAS PULLING ROOF BOLT OFF PINNER AND HAD HIS HAND ON END OF BOLT. RAN BOLT INTO RIB WITH HIS HAND ON THE END, SMASHING END OF FINGER.

He was helping put cribs blocks under the bumper of the miner, he left his right hand on a crib block as the operator lowered his stab jack, causing his two fingers to be smashed.

HE WAS PINNING AND REACHED DOWN TO PICK UP A PIN WITH HIS RIGHT HAND. HE WAS HOLDING ONTO HIS WRENCH,WHICH WAS IN THE CHUCK OF THE BOLTER WITH HIS LEFT HAND. APIECE OF ROCK FELL OUT STRIKING LEFT HAND.

HE WAS GOING TO PUT UP ROOF BOLT, WHEN A CO WORKER NOTICED LOOSE ROCK OVER HEAD. EE SWUNG BOOM BACK IN AND STEPPED BACK BESIDE BOLTER, WHEN THE ROCK FELL AND STRUCK HIM ON RIGHT SIDE.

HE WAS OPERATING DOUBLE BOOM PINNER. HE LOOKED OVER TO CHECK ON CO-WORKER, WHEN HE ACCIDENTLY HIT LEVER TO MAKE BOOM COME DOWN ON HIS FOOT.

WHILE HE WAS CUTTING A TIMBERS, A PIECE SLID OFF THE BUCKET AND STRUCK HIS RIGHT FOOT.

HE WAS PINNING WHEN GOB FELL OUT ON HIS RIGHT FOOT.

HE WAS STARTING STEEL WHEN A PIECE OF ROCK BROKE OFF AND HIT HIM IN THE MOUTH.

HE WAS RUNNING THE MINER, WHEN A SMALL PIECE OF SLATE ROCK FELL OUT BETWEEN PINS, STRIKING HIS RIGHT HAND AGAINST THE REMOTE BOX CUTTING HIS HAND.

HE WAS STARTING TO DRILL 1ST HOLE IN FACE, WHEN GLOVE GOT CAUGHT ON PINNER STEEL CAUSING HIM TO HURT HIS WRIST. FINGER.

HE WAS PINNING WHEN TOP FELL OUT BETWEEN PINS, ROCK STRUCK EE ON BACK.

HE WAS BOLTING TOP WHEN HIS GLOVE GOT CAUGHT AROUND THE STEEL, SPRINGING HAND BACK (RAVELING ON NEW PAIR OF GLOVES GOT CAUGHT ON STEEL..)

HE WAS MOVING CANOPY TO INSTALL 2ND BOLT, WHEN LOOSE GOB FELL OUT AND STRUCK HIM BEFORE HE WAS ABLE TO GET UNDER CANOPY. THE ROCK HINGED OUT OF CANOPY BEFORE HITTING HIM. THE CANOPY WAS PRESSU RIZED AGAINST THE TOP, DISLODGING THE ROCK.

EE HAD BENT A STEEL, PULLED IT OUT. HE PUT A NEW STEEL INTO HOLE W/BOTTOM SECTION. HE WAS UNAWARE THAT A BIT WAS LEFT IN THE HOLE. HE STARTED DRILLING, STEEL BROKE & THE BOTTOM SECTION HIT HIM IN THE FACE.

WHILE INSTALLING ROOF BOLTS IN THE BOTTOM STATION THE EMPLOYEE SMASHED HIS FINGER BETWEEN THE DRILL HE WAS OPERATING AND A STEEL POST.

ROCK 10"X 8"X 1/2" FELL OUT BETWEEN PINS, STRIKING HIM ON THE HEAD.

HE WAS PUTTING UP X-OVERS. HE WAS REPOSITIONING THE PINNER WHEN HE CAUGHT HIS THUMB BETWEEN BOOM AND PINNER STEEL.

HE WAS PINNING IN THE FACE, KNEELING DOWN ON KNEE WHEN A ROCK FELL OUT HITTING HIM ON RIGHT MIDDLE FINGER.

THE EMPLOYEE WAS OPERATING A 630 MUCKER. A ROCK FELL OUT OF THE BUCKET AND STRUCK HIS LEFT RING FINGER, CAUSING A LACERATION AND BREAKING THE BONE.

HE WAS DRILLING A HOLE AND MASHED HIS FINGER BETWEEN THE BOOM AND THE TOP.

HE WAS OPERATING THE MINER, WHEN HE BUMPED HIS ELBOW ON THE CORNER OF THE MINER. OBSERVATION DAY 2/14/01 WENT TO DOCTOR DRAINED FLUID.

EE WAS PUTTING UP CROSSOVERS WITH SINGLE BOOM

THE BOLTER KICKED REEL IN ON SINGLE BOOM, AND THE CABLE JERKED HIS FOOT OUT FROM UNDER HIM.
