Showing all 15Employee had been cutting and welding and a small piece of metal got into his eye. He was wearing safety glasses and face shield. He was prescribed eye drops and ointment for his eye.
Employee was cutting off the right side of the truck bed. Had already removed 2/3 of right side. Cut 3 of the 4 wearbars/gussetts. Employee then moved back to the outside of the bed to continue cutting when the port he was working on broke loose and fell striking employee and pinning him on the right side leg and arm along with his left foot.
Using a one inch impact wrench to remove R.O.P.S. bolts on 992G. When bolt broke loose, EE threaded upwards and smashed left hand between wrench & angle above it.
Employee was running a Caterpillar D10R Dozer. As he was backing up, he struck a 3' high pile of coal which gave him a jolt. Employee claims he began to see a black spot in his right eye.
EE was using a handheld grinder in a tight position and got something in his eye. EE was wearing safety glasses with side shields at the time.
Employee was operating a mobile crane looking up often to watch the loads, and over the course of time, his neck and shoulders began to hurt.
Member was operating d10 dozer in 15' shot material and backed into a large rock with right track causing him to jar his lower back.
EE WAS TRAMMING A 16G GRADER TOWARD THE PARKING LOT WHEN THE MEMBER RAN INTO A SUMP AND EJECTED FROM THE GRADER
ON 9/20/04, EMPLOYEE SOUGHT MEDICAL TREATMENT FOR LOWER BACK PAIN. HE DOES HAVE A PRE-EXISTING BACK INJURY. HE WAS LAST OFF WORK ON 9/14/00 TO 11/06 DUE TO HIS CONDITION. HE BACKED OVER A ROCK IN A CAT D8R DOZER WHICH IRRITATED HIS PRE-EXISTING INJURY. HE IS CURRENTLY UNDER A PHYSICIAN'S CARE. NO DEFINITIVE DIAGNOSIS HAS BEEN MADE AT THIS TIME.
Employee was struck by jib being installed on a Grove 65 ton Chase to add reach. as it was swinging into position, he grabbed it to slow it down with another ee. It struck something and bounced back, hitting rim in the left side of the chest.
WHILE VACUUMING COAL AROUND #3 FEEDER, THE FENCE GUARD SLID DOWN ON EE S LEFT HAND, CUTTING MIDDLE FINGER REQIREING 10 STITCHES EE WAS WEARING ALL REQUIRED P.E.
HAND CAUGHT IN ELEVATOR DOOR, FRACTURE OF MIDDLE FINGER ON RIGHT HAND. HE WAS TRYING TO KEEP THE ELEVATOR DOOR FROM CLOSING.
EE WAS SETTING UP CRANE, WHEN A SKID PLATE FELL FROM OUTRIGGER JACK. SKID PLATE STRUCK HIS MIDDLE FINGER ON HIS LEFT HAND. EE SUFFERS FROM A BROKEN FINGER.
EMPLOYEE WAS CUTTING WITH TORCH OVERHEAD WHEN A HOT PIECE FELL INTO SHIRT SLEEVE AND BURNED HIM ABOVE THE RIGHT WRIST. THE BURN GOT INFECTED AND HE HAD TO GO FOR TREATMENT 12/4/01.
