IE was cleaning the inside of the rotary breaker with a pneumatic chipping hammer when he stepped back and lost his footing. IE said he reached for the breaker plow to brace himself and pinched his finger between the pneumatic chipping hammer and the plow. IE was diagnosed with a broken finger and the finger nail was removed to relieve pressure.

Employee states he was installing a resin roof bolt. He bent the bolt, inserted the glue tube, then pushed the bolt in the roof. He failed to completely insert the bolt past the bend, and raised his boom and started to spin the bolt prematurely. The bolt plate spun around and cut his left elbow. The cut required 5 sutures.
EE was assisting with grinding, Employee felt like he had something in his eye, employee stated he did not know if it happened at work or at home.
Employee stated he was bolting and had dropped his boom down against the floor, with pressure on it. He then lowered his canopy and raised the boom off the floor at about the same time. This caused the canopy to come down faster than he thought and it struck him on top the hard hat. He worked until 06/24/2010 when taken off by his doctor.
Employee stated he was operating the Cont. Miner and had just set the machine over in the cut and was waiting on a car to come in to load. He stepped up beside the machine and looked back to watch for the car coming in, when a piece of slate slid off the top on the machine and struck his foot. He finished his shift, but was taken off work on the 29th of March.
Employee stated he was roof bolting on his knees and when he swung the boom over to install a rib pin he lost his balance. When this happened he grabbed the controller in such a way that he let the drill pot down on his leg/knee. Employee was taken to medical services and reported to have a severe contusion to his Lower thigh and knee.
Employee was swinging bolter boom over to install rib bolt when the boom pushed up a piece of rock and struck employee's left ankle. His foot was in a hole when he started to swing the boom.
Employee was using a hydraulic wrench to tighten bolts on brealer plate at the rotary breaker. The location of the bolt was over his head and behind access door. The tool was slipping off the nut, when pressure would release. Employee's hand slipped a little enough while holding tool and his thumb came between the edge of the reaction arm of the tool and the I-beam.
Employee was drilling a hole in the roof with a roofbolter when a piece of rock broke loose and slid down the drill steel. The rock flew off of the drill steel and struck employee in the mouth causing him to receive stiches in his bottom and top lips.
Employee was drilling a hole in the roof with a roof bolting machine. As he was retracting the drill steel out of the roof a piece of rock slid down the drill steel causing a laceration to the his right upper arm. He did not miss anywork but did require stitches.
Employee stated he was driving the slinger duster down a travelway, when he thinks he hit a hole and caused him to be bounced up and hit his head on the canopy. He felt in his neck and head. Employee had prior neck surgery and the MRI showed degenerative stenosis and cervical spondylosis. Employee was taken off work on 02/20/2009.
Employee stated he was bolting and was straightening a 6' bolt for installation when it slipped in the grip of his hand. The bolt spun around causing the employee to jerk and fall foward injuring his shoulder.Employee started missing work on 5/7/09 by 220091320026
Employee stated he had walked up to the Bolter to cut a few pieces of tie wire. The Bolters were installing bolts and as the injured employee reached to cut the wire, the bolter operator trammed foward and onto this fellows foot. Slight hair line fracture to his foot. Started missing time the next day 11/22/2008.
Employee was a Roof Bolter and stated that he was picking up and loading some roof plates onto his boom tray, while his buddy was installing a corner bolt. When the installation was done that person went back and let the ATRS down a little, causing the bolter to roll backwards onto the victims foot.
Employee stated the operator told him he was starting the roof bolter and for him to pull foward and before he could get out of the way, the operator trammed the bolter foward running, over his toes.
Employee stated that he had installed a bolt close to the rib and had let the pressure off his canopy to swing his boom over toward the center. The canopy didn't come down, it was hung on the rib so he released his floor jack to swing the boom over, not realizing what could happen. The canopy when he started swinging, free fell and struck him in the head, neck and back.
Employee stated he was using a bolter, installing chain hangers in a new cut out drive area. The top was higher than normal and he was using a longer steel to drill the hole, when it hit a crack in the strata, jumped up, started to wobble. He reached out to grab the steel and also hit the up pressure at the same time and the steel bowed and struck the back of his hand.
Employee was tramming the roof bolter backwards thru a line curtain. He had his hand and arm reaching out behind him pushing the curtain back. The bolter machine went over a hump and the cab/canopy of the machine went up to the roof. When it did this his hand was pulled on top of the canopy and his finger was caught between a curtain board and the canopy.
Employee stated he was installing a Tail lift jack on a cont. miner. Two other people were holding the new jack up with a bar. The bar was ran thru the wire that holds the jack in the collapsed position. The injured had placed his hand in the jack holder trying to get some excess gob out of it. Wire broke, jack expanded and caught his hand.
Employee was roofbolting in the 5L entry. Employee had set the foot jack of the roofbolter on a piece of draw rock (unknowingly). When he started to drill the rock broke causing the atrs and the canopy to come down striking the employee on top of the head.
Employee stated that he was using a torch cutting an Axle Pin out from underneath a Shuttle car. A piece of hot slag popped off the pin and went into his ear. He has not missed any work. Started missing 11/27/2006 with surgery to repair a hole. This was not turned in to me until 9/27th.
Employeewas inside a cyclone overflow box using a straight arm chipping hammer. He was in a congested area with restrictions of movement and was having to apply pressure on the hammerwhile chipping at the ceramic. The chisel slipped and his right index finger struch a piece of sharp ceramic causing a laceration.
Employee was roof bolting using 6' bolts. While bending the bolt for installation he felt pain in his shoulder.
EE was using a cutting torch, laying on his side, cutting on a beam at the #1 conveyor Belt Tail. A piece of hot metal flew into EE's ear. He did not fill out an acc. report at that time. He started having trouble with the ear, so he went to the doctor & did not fill out a report until 3/29/05. He also did not miss any work until 4/21/05. Surgery, hole in eardrum.
EE WAS OPENING A BAG OF ROCK DUST LOCATED ON THE BACK OF THE BOLTING MACHINE WHEN THE OPERATOR STARTED UP THE BOLTER AND TRAMMED FORWARD, RUNNING OVER EE'S FOOT.
Employee was installing a roof bolt that was bent a little in the head end. He slipped, causing him to hit the rotation level before he was ready. He had a hold of the pin and it spun around out of his hand and struck him on the thumb/wrist. He didn't miss work until 11-16-04.
EMPLOYEE WAS TRYING TO FREE UP TWO SECTIONS OF DRILL STEELS THAT WERE HUNG TOGETHER. HE WAS MOVING THE DRILL BOX IN AND OUT USING THE BOOM EXTENSION LEVER. HE MISSED THE EXTENSION LEVER AND HIT THE BOOM SWING. HIS HAND WAS ON THE DRILL STEEL AND WHEN HE HIT THE WRONG LEVER IT SWUNG THE CANOPY INTO HIS ARM, PINCING IT BETWEEN THE CANOPY AND DRILL STEEL.
EE WAS ROOFBOLTING & WAS BENDING A 6' BOLT TO STRAIGHTEN IT FOR INSTALLATION. THE BOLT TURNED & SPUN AROUND WITH HIM AS HE WAS ATTEMPTING TO BEND IT.
Employee was hanging roof bolter cable, while the bolter was moving. The bolter ran out of cable and jerked the cable down from the roof, striking the victim and knocking him down. Employee didn't miss work until 11-1-04.
EMPLOYEE WAS INSTALLING A ROOF BOLT AND WHEN HE REACHED HIS ARM AROUND TO THE CONTROLS HE STRUCK HIS ELBOW ON THE CANOPY JACK STAND HOUSING.
WAS ROOF BOLTING AND WAS DRILLING A HOLE IN ABOUT 10 FT TOP. HAD DRILLED WITH HIS FIRST SECTION OF STEEL AND HAD WENT TO PUT 2ND SECTION ON WHEN THE FIRST SECTION FELL OUT OF THE HOLE AND STRUCK HIS RIGHT INDEX FINGER BETWEEN IT AND THE PINNER WRENCH.
Ee was helping hang the miner cable. Miner operator trammed the miner forward while they were hanging cable and it pulled too tight causing it to fall while ee had a hold of it. Bent his wrist back.
EMPLOYEE WAS ROOFBOLTING & HAD BENT OVER TO PICK UP A ROOF BOLT FOR INSTALLATION AND WAS STRUCK BY A ROCK THAT FELL FROM AROUND THE ATRS.
HE HAD STARTED A HOLE BESIDE A SULPHER BALL, WHEN THE BALL SHATTERED. A PIECE OF ROCK FLEW BACK AND STRUCK HIM ON HIS RIGHT INDEX FINGER. HE SAID HIS HAND WAS DOWN BY HIS SIDE, WHEN IT WAS HIT BY THE PIECE OF ROCK. MET. GLOVES WERE ALSO WORN.
EMPLOYEE WAS ROOF BOLTING AND HAD INSTALLED A ROOF BOLT AND WAS IN THE PROCESS OF LOWERING THE BOOM. AS THIS WAS BEING DONE, HE TURNED & SLIPPED WITH HIS KNEE GOING UNDER THE BOOM.
EMPLOYEE WERE BACKING ROOF BOLTER ACROSS AN ENTRY. OPERATOR TURNED ON THE REEL TO REEL EXCESS CABLE UP. REEL PRESSURE WAS SET TOO HIGH AND IT JERKED THE HUNG CABLE DOWN STRIKING VICTIM.
EE WAS DRILLING A ROOF BOLT HOLE BESIDE A SULFUR BALL. THE BIT HIT THE HEAD AND IT FELL ON THE DRILL POT & THEN HIT VICTIM ON FOOT.
EMPLOYEE WAS ROOF BOLTING AND HAD INSTALLED HIS FIRST BOLT AND WAS IN THE PROCESS OF INSTALLING HIS SECOND BOLT IN PATTERN. HE HAD TURNED AROUND TO PICK UP HIS BOLT AND A ROCK FELL FROM THE RO OF AND RIB STRIKING HIS BACK AND SHOULDER.
EE WAS ROOF-BOLTING & IN THE PROCESS OF DRILLING A HOLE. HE HAD DRILLED UP ENOUGH TO INSTALL HIS SECOND SECTION OF DRILL STEEL. THE STEEL DIDN'T SEAT INTO THE CHUCK AND HE HAD PRESSURE ON IT, WHICH CAUSED IT TO BOW. HE REACHED FOR IT WITHOUT TAKING PRESSURE OFF. THE STEEL SPRANG OUT STRIKING HIS LEFT INDEX FINGER.
EMPLOYEE WAS ROOF BOLTING WHEN HE NOTICED SOME LOOSE ROCK. HE TURNED AROUND TO GET A PRY BAR FROM THE TOP OF THE BOLTER. THE ROCK FELL AS HE WAS TURNING AROUND & STRUCK HIM ON THE BACK OF THE HEAD.
EE WAS STARTING TO INSTALL HIS FIRST BOLT OF THE SHIFT, ON A CORNER. CANOPY WAS UP AND A ROCK FELL FROM THE ROOF, HIT THE CANOPY, SWUNG DOWN & PINNED VICTIM UP AGAINST CONTROLS & MACHINE.
EMPLOYEE WAS OPERATING A CONTINUOUS MINER, WHEN A ROCK FELL FROM BETWEEN THE BOLTS, STRIKING HIM ON THE FOOT.
EMPLOYEE WAS OPERATING A CONTINUOUS MINER BY REMOTE. HE WAS CUTTING THE FACE OF #2 ENTRY. ROCK FELL OUT STRIKING HIS HAND.
EE WAS ROOF BOLTING. HE WAS PUTTING THE DRILL STEEL INTO WRENCH WHEN HE APPLIED PRESSURE BEFORE THE STEEL WAS LOCKED INT HE WRENCH. IT SPRUNG OUT, BENDING THE STEEL AND HITTING EE IN THE HAND.
EMPLOYEE WAS REMOVING BENT PIN, WHEN IT STUCK HIM IN THE FACE.
EMPLOYEE WAS INSTALLING A CURTAIN BOARD AND GOT MIDDLE LEFT HAND FINGER CAUGHT BETWEEN TOP AND BOARD.
EMPLOYEE WAS PREPARING TO PUT OIL IN MACHINERY, TURNED ON POWER THE CABLE REEL WAS STILL ENGAGED, CAUGHT HIS LEGS PULLING HIM INTO THE FRAME.
EMPLOYEE WAS ROOF BOLTING. ROCK FELL FROM FACE, STRIKING LEFT ANKLE.
EE GOT HIT BY FALLING ROOF ROCK, WHILE CUTTING RIGHT SLIDER.
