EMPLOYEE WAS USING A BENCH MOUNTED BELT SANDER AND A VERY SMALL METAL PARTICLE FLEW INTO HIS EYE. THE EYE CLINIC REMOVED THE OBJECT WITHOUT COMPLICATION.

Employee was removing a motor from dragline. Another ee was using hoist to lift the motor. EE reached into the area to remove shims. When he did this, hoist operator moved the load resulting in EE getting his finger caught between the side of the foot and steel plate.

The ee was operating a cat truck excavator doing clean-up work in the coal pit. When a soocer ball sized rock came off the highwall and broke through the cab window. Broken glass or a fragment from the broken rock caused a 2" laceration on the right side of the ee's head. He was transported to the hospital ER where 7 staples were required to close the wound.

THE EE WAS DOING REPAIR WORK ON RUBBER CURTAINS AT THE COAL TRUCK DUMP. AS HE HELD THE MATERIAL TO STEADY IT WITH HIS LEFT HAND, HE BEGAN TO DRILL INTO RUBBER BELTING. AS THE DRILL BIT PENETRA TED THE RUBBER, IT CONTACTED THE INDEX FINGER ON THE LEFT HAND. SIXTEEN STITCHES WERE REQUIRED TO CLOSE THE WOUND.

THE EE WAS LOOKING FOR THE SOURCE OF SMOKE AFTER WELDING OPERATIONS ON A 2800 SHOVEL. HE FELT HEAT ON A MANDOOR AND WHEN HE OPENED THE DOOR FLAMES AND SHOT OUT AND BURNT HIS ARMS AND FACE.

WHILE MOVING HYDRAULIC JACK TO POSITION TO LIFT ENGINE ON 285 DEMAG HYDRAULIC SHOVEL, MECHANIC WAS STRUCK ON THE BACK OF THE HEAD WHEN THE JACKSLIPPED AND FELL. THE IMPACT KNOCKED THE MECH- ANIC OFF OF THE LADDER HE WAS STANDING SEVERAL STEPS UP ON.

EE WAS REPLACING BUCKET LATCH BAR ASSEMBLY ON A P&H SHOVEL. A BLOCK WAS BEING USED TO HOLD THE 300 LB LOAD. NEAR THE END OF THE TASK, THE BLOCKWAS REMOVED BUT THE LOAD WAS STILL HELD BY A CHA IN. HE REACHED INTO A PINCH POINT AREA. THE LOADSHIFTED AND DROPPED 6" MASHING HIS LEFT INDEX FINGER.

EE WAS USING A 1/2" AIR IMPACT WITH A 1/2" HEX HEAD SOCKET TO REMOVE ALLEN HEAD BOLTS. WHILE DOING SO THE ROLL PIN WHICH RETAINS THE HEX TO THESOCKET WORKED OUT. USING HIS LEFT HAND TO STABIL IZE & GUIDE THE SOCKET & BOLT, HE RECEIVED A LACERATION TO HIS LEFT PALM WHEN THE ROLL PIN WORKED LOOSE.
