Showing all 26Employee was bolting in the #1 heading. When he went to move the next bolt he had placed his left hand on the canopy jack. Then he grabbed the wrong lever letting the canopy jack down on his little finger.
EE was operating roof bolter while ee was drilling a test hole he put on a second piece of steel. When ee finished the hole the roof bit got hung in the top causing the connector between the 2 pieces of steel to break and let the second piece of steel strike the ee on the left hand and index finger causing a bruise to the hand.
Employee was bending a roof bolt when a small piece of drawrock fell against t bar causing him to jerk back. Sudden motion caused severe pain in lower back and testicles.
Employee was drilling a hole to install a roof bolt when his glove got caught on the drill steel, which caused his hand to twist with the drill steel before he could stop the rotation of the steel or get the glove off.

Employee was tramming miner when he slipped & fell catching himself with his right hand causing a strain to his right shoulder. Employee did not miss any work due to this injury until 2/8/06 - Had an MRI on 2/8/06.

EMPLOYEE WAS INSTALLING A ROOF BOLT AND WAS ATTEMPTING TO PUSH THE BOLT THROUGH TWO ROOF MATS WHEN HE FELT A SHARP PAIN IN HIS RIGHT ELBOW.
EE WAS STARTING TO DRILL A HOLE TO INSTALL A ROOF BOLT. THERE WAS SOME COAL ON THE MINE ROOF & WHEN HE STARTED THE ROTATION, A PIECE OF IT BECAME DISLODGED STRIKING HIM IN THE RIGHT EYE. EE ST ATED HE HAD SAFETY GLASSES ON.
EMPLOYEE WAS ATTEMPTING TO PUSH A PIECE OF DRILL STEEL UP TO DRILL A HOLE. HE HAD HIS HAND ON THE STEEL, WHICH WAS NOT ALL THE WAY IN THE DRILL CHUCK, WHEN HE APPLIED TOO MUCH PRESSURE CAUSING THE DRILL STEEL TO SLIP AND VIBRATE INTENSELY BREAKING HIS THUMB.
EMPLOYEE WAS ATTEMPTING TO PULL A ROOF BOLT FROM THE MINE ROOF THAT WAS NOT PROPERLY INSTALLED WHEN THE BOLT BECAME DISLODGED STRIKING EMPLOYEE ON LEFT FOREHAND.
EMPLOYEE WAS OPERATING MINER WHEN A PIECE OF MINER BIT BROKE PUNCTURING EMPLOYEE ON BACK OF RIGHT HAND. HAD A PIECE OF METAL REMOVED FROM HAND & RECEIVED 2 STITCHES.
EE STATED A BLOCK OF COAL FELL OFF MINE RIB STRIKING HIM ON THE BACK AND SHOULDER.
EE WAS REMOVING DRILL STEEL OUT OF ROOF WHEN PUSHER STEEL FELL OUTOF HOLE STRIKING HAND AREA FRACTURE SMALL RIGHT FINGER
EMPLOYEE WAS ROOF BOLTING AND GOT HIS LEFT HAND CAUGHT BETWEEN 2 PIECES OF DRILL STEEL.
EMPLOYEE WAS INSTALLING A ROOF BOLT WHEN HE CAUGHT HIS GLOVE IN BETWEEN THE BOLT WRENCH AND ROOF BOLT. HE SAID HE JERKED AWAY AND FELT SHARP PAIN IN HIS SHOULDER.
INSTALLING ROOF BOLT, SPINNING BOLT PLATE CAUGHT LEFT ARM ABOUT 4" ABOVE WRIST CUTTING ARM.
