Showing all 41Employee was operating a road grader blading a haul road, backed the machine up and ran over a large rock jarring the operator which caused the operator to experience pain in the neck and shoulder area. The doctor diagnosed this as a neck strain/sprain. Prescribed pain medication and physical therapy.
Employee was doing a clean-up in front of the shovel, when the moldboard caught a rock stopping the blade's forward motion jarring the employee's neck and back.
After power washing trucks, employee complained of left wrist pain
The 651 scraper traveling down a ramp at approximately 8 m.p.h. Scraper started riding up the side of the parallel berm, went over the top of the berm, and down the other side 30 feet stopping at the bottom of the other side. The machine had no damage and the employee was wearing his seat belt.
While operating haul truck, & while loading under a shovel, haul truck opr leaned over in the seat to grab a roll of paper towels & hit her head on gear lever when a chunk off the bucket contacted truck bed. 8 days later, after experiencing headaches & vision problems, the opr was diagnosed w/a concussion resulting from contact w/gear selector. Mine rec'd documentation on 2/4/14.
While employee was operating shatter box, he was distracted and inadvertently placed his hand in a pinch point, resulting in a laceration with finger tip fracture. Employee had not followed procedure and did not close the lid to the box while it was in operation.
Possible bump on fill surface. No equipment damage. Possible kink in neck.
EE went to talk to anther EE that using a drill. EE felt something enter their eye when they were walking up to or standing by the EE using the drill. EE was given prescription medication making it a MTC.
Pneumatic hand grinder slipped and cut ee left knuckle.
Small piece of metal in eye while grinding. Was Wearing goggles and face shield.
EE was taking a break when they noticed something in their eye from welding and grinding early in shift. EE was given prescription medication and had a rust ring removed.
Employee was performing repairs on a shovel revolving frame during air arcing and welding metal particles entered his eye.
2 Employees were welding on same piece of metal within 5' of one another under silo #2. One employee received flashburns to both eyes.
The welder was preparing to lift crane outrigger pads from the front of the crane when he was struck by the crane. He sustained a fractured shoulder blade.
A dozer operator was pushing coal cleanings into a berm in a drilling and blasting area. While backing to reposition, the operator misjudged her distance from the crest and backed off the highwall. The dozer fell approximately 33 feet onto a slough pile. The operator was dignosed with one ruptured disc.
An operator was moving a D11 Caterpillar dozer from one work area to another. As he was traveling the dozer encountered an abrupt change in the surface elevation. The sudden movement caused the operator's right hand to be pinched causing a small fracture in the tip of the right thumb, and stitches to close the wound.
While using a jack to change a tire the tech pinched his thumb between the jack and the truck tow hook.
While cutting cable tray with a battery powered sawsall, the individual's left hand slipped past the tool grip, crushing his finger in the mechanism driving the saw blade. Stitches were required.
While using a floor mounted grinder to smooth up a cut on a thin piece of metal the object and the employees right hand were pulled into the wire brush wheel. The table and guards were all in place and properly adjusted. The employee sustained an amputation on the tip of the little finger and a laceration on the ring finger which required 12 stitches to close.
Conditions unknown. Minor right eye irritation.
Mechanic was straightening sheet metal on a Terex MT5500 haul truck with a small porta-power. The porta-power ram slipped and pinched thumb against sheet metal.
A welder was attempting to attach a 3/16 steel sheet. The steel slipped 14 inches resulting in a laceration requiring stitches. The welder was wearing leather gloves at the time of the incident.
Grinding particulate entered left eye sometime during process of welding, grinding, switching PPE, and moving place to place.
At the time of the incident the technician was removing the wheel bolts from a CAT 793 truck. When the technician engaged his impact wrench, the socket came free from the wrench and struck him in the upper lip.
While cutting conduit with a battery saws-all, the tool slipped and injured his right thumb causing a minor crushing injury to his thumb.
Truck operator sustained a neck and shoulder strain when the bottom of an electric shovel door contacted the truck bed.**It was originally believed that this injury was minor and 1st aid only. Injured had ongoing discomfort and saw a Dr. and on 8/142007 was restricted from returning to work.**
While cutting angle iron on guard, grinding wheel caught on steel, kicked back & cut finger with cutting disc.
As EE was coming down the front ladder of a CAT 793 truck, a welder working 15 feet away struck an arc, causing him to see "spots". The next morning his eyes were swollen and felt like they had something in them. He went to the doctor and was diagnosed as having corneal ultraviolet burns,
At the time of the incident employee was installing a final drive cover on a CAT 793 truck. As he was using an overhead hoist to move the cover, the threads on the bolt attaching the lifting link to the cover stripped, allowing the cover to drop and strike employee on his right foot.
While hoisting the shovel bucket up the wall to fill the bucket, a chunk of dirt cascaded over the shovel bucket, rolled down the shovel boom, through the safety glass, and entered the cab. The shovel operator was treated for a foot injury resulting from an unknown source.
Welder was cleaning up the scrapes & gouges on an 18" piece of aluminum bar stock using a grinder. A shovel operator was assisting the welder & was positioned behind & to the right-holding the loose end of bar with his rt hand. Grinding wheel stalled in a gouge on bar causing grinder to rebound. The force caused the welder to fall back. Grinder then struck shovel operator's arm.

MAINTENANCE EMPLOYEE WAS GRINDING METAL DURING A SHOVEL REPAIR. AFTER EMPLOYEE FELT IRRITATION IN HIS LEFT EYE, DEBRIS WAS REMOVED.
Ee walked around the corner of a electrical box in the shop. another ee was grinding on it and a piece of slag went into first ee's right eye.
THE EMPLOYEE WAS LANCING A ROLLER PIN IN ORDER TO REMOVE IT. THE HEAT FROM THE LANCE IGNITED GREASE AND THE FLAME WAS FORCED OUT OF THE HOLE, KNOCKING DOWN THE EMPLOYEE AND BURNING HIS FACE.
GRINDING ON HORSE CALLER, GRINDER KICKED BACK & FRACTURED LEFT THUMB.
EE WAS LEANING BACKWARD WELDING OVERHEAD AND THENEXT DAY HAD A SORE BACK.
THE EE WAS USING A HAND GRINDER TO CUT THROUGH A WELD THAT WAS SECURING AN IRON BRACE IN PLACE. AS HE CUT THROUGH THE WELD, A 3/8" STEEL PLATE THAT WAS BEING HELD BY THE IRON BRACE, FELL AND H IT THE EE ON HIS LEFT WRIST.
EMPLOYEE WAS SITTING IN A HAUL TRUCK UNDER A SHOVEL BEING LOADED WITH DIRT; AS THE SHOVEL BUCKET WAS DUMPING A LARGE CHUNK OF CLAY INTO THE TRUCK BED, IT CAME APART, HITTING THE INSIDE OF THE TRUCK BED & JARRING THE EMPLOYEE AROUND IN HER SEAT AND HURTING HER BACK AND NECK. DOCTOR PUT HER ON LIGHT DUTY FOR 5 DAYS.
