Employee was operating the pan bolter at shield #121 on the 8th South longwall face. As EE attempted to raise the mast, EE inadvertently activated the clamps and pinched EE's left thumb and index finger resulting in lacerations requiring stitches and a fractured bone in EE's index finger.

While operating the tailgate drum on the shear at shield #30 on the 8th South Longwall, The employee was struck in the back of the head by a lump of coal that had flown over the raise sloughing plate on the shear. Employee received a laceration which required medical treatment (Staples).

Employee was hit in the face shield of helmet while operating the headgate cutter drum at shield #104 on the 8th South longwall face while employee was checking the flow of coal in the face conveyor.

Employee was roof bolting in the 1st North Rock Slope, employee was putting a 5 foot pusher steel into the 3 foot extension steel and as employee was holding the chuck wrench while pushing the steels into the hole, employee smashed right little finger between the chuck wrench and the clamps on the drill head of the roof bolter.

Employee was standing in the walkway at shield #10 and as the pan was being advanced, an anti-slew jack got hung up on the bears ear bracket and when it popped free the slew jack stuck the employee in the right knee. EE went back to the Dr. on 4/9/2018 and was given a work release for modified restrictions.

Employee was operating the tailgate drum of the shear at mid face grading bottom while cutting towards the headgate. The employee placed left hand on top of the spill tray and a rock was kicked up by the cutter drum and landed on top of employee's left hand causing a laceration which required stitches.

While operating the left side drill head of the RB-2 roof bolter, the starter steel became stuck in the mine roof. The operator lowered the drill box down 1/2 of the way and jerked the stuck steel out of the hole. The operator's small finger on left hand was pinched between the drill steel and the drill head of the machine. 3 stitches were required to close the wound.

A contract miner employee was injured at shield #41 on the longwall face when EE got right foot smashed with a slew jack as the face pan was being pushed forward suffering a contusion to the top of right foot.

The shear was cutting up hill at the headgate into the web for the push over. Employee was standing behind the shear at shield #20. The sloughing plate on the shear was in the raised position. Coal and rock was expelled from the face/roof onto the shear and bounced over the sloughing plate and hit employee in the right shoulder and air stream helmet.

Employee had finished drilling a hole and had inserted a tube of resin and a bolt into the hole. As EE was pushing the bolt up to set it, the hinge pin for the drill mast fell out allowing the mast to lower and tilt to one side. The drill mast contacted a drill steel in the steel tray and flipped it up and struck the EE in the left ear, requiring stitches.

Multiplying tailgate drum bolts, multiplier malfunctioned, ripping it out of the right hand, swinging around and hitting the left hand just above the first knuckle.

There were 4 employees trying to get the panbolter up to the tailgate. It kept getting hung up on the spilltray. The employee tried to place a cap piece between the pan bolter and the spilltray. The panbolter moved and swung back against the spilltray hitting the cap piece, causing the cap to pinch the employee's left thumb against the spill tray.

Operator was changing steel in drill box. He reached back to lower the box, and pulled the lever the wrong direction raising the box and bending the steel. The steel pinched his finger between the steel and the lifting cylinder.
