Hydraulic oil sprayed out of the boom lift jack of the roof bolter, spraying oil in miner's face, getting in his eyes. Miner was wearing glasses.

EE WAS HANGING OUTBY END OF WING CURTAIN IN #6 ENTRY. LEFT BOLTER WAS IN LAST OPEN BETWEEN 6-7. BOLTER CABLE WAS HUNG ACROSS #6 ENTRY OVER HEAD OF HELPER WHERE HE WAS HANGING CURTAIN. BOLTER O PERATOR STARTED HIS MACHINE NOT KNOWING HIS REEL WAS ENGAGED. CABLE PULLED LOOSE FROM WIRE IT WAS HUNG WITH KNOCKING EE TO GROUND. HE FELL ON HIS RT HAND, INJURING HIS THUMB.

ROOF BOLTER WAS BEING MOVED FROM RIGHT TO LEFT SIDE. BOLTER TRAM WAS UP ON A LUMP OF COAL. WHEN THE BLOCK OF COAL BROKE, THIS ALLOWED THE PINNER DECK TO FALL DOWN AGAINST HIS RIGHT KNEE.
