The employee was installing a canopy jack on the roof bolter. He placed the canopy jack on the front of the roof bolter and the jack slid off and hit him on the right foot and ankle. This caused a bone fracture under his ankle.
Employee was helping miner man move miner. While pulling cable, the employee bent over to grab a pull rope. Resting his right hand on top of the miner, the miner began to tram causing the employee's right hand to be pinned against the top.
Piece of rock fell and hit employee on shoulder.
Rock fell from roof 12' high, struck ee in head and shoulder forcing him to the ground, he has cuts on his face and forehead and complaining about shoulder hurting.
THE EMPLOYEE WAS STRUCK IN THE LOWER BACK BY A SLAB OF ROCK.
WHEN THE EMPLOYEE WAS REACHING FOR THE REMOTE CONTROL UNIT A PIECE OF DRAWROCK FELL FROM THE ROOF AND STRUCK HIS LEFT HAND. THIS RESULTED IN A LACERATION TO HIS THUMB. THE EMPLOYEE DID NOT NOT IFY OPERATOR OF RECEIVING STITCHES UNTIL 5-9-01.
