Employee was pulling a shield in on the longwall face. In the process of pulling in the shield his foot caught got under the pontoon of the shield and it caused him to smash his right foot. It broke the tips of three of his toes and the employee had to receive four stitches in one of them.

Employee was standing beside the C/M operator while he was cutting out a belt channel on 16 Headgate. When the C/M operator was cutting some top, a piece of a roof bolt came back and struck the employee in the mouth.

Employee was installing roof bolts down the face of the long wall in preparation of moving the long wall. Employee was trying to position the roof bolt plate between the shield and ATRS. The bolt was pushed up, smashing the tip of his left point finger between the roof bolt plated and mine roof.

Employee had his left knee resting on a relay bar at shield #17 when someone pushed the panline causing the relay bar to jump up and the employee sprained his left knee. *Employee was not removed from work due to injury until June 3, 2014.*

Employee was bolting in No 6 entry. As he was drilling a hole, his pusher steel got hung and flew out striking employee above his left eye.

Employee was bolting on 8A section. He was backing his drill steel out of the hole when the pusher came apart from the finisher. The pusher steel came around and struck the employee on the right side of his face.

EE was cutting, welding and grinding on a scoop frame and some time during the process a sliver of metal became embedded in his left eye.

Shuttle car operator pulled under boom of the continuous mining machine. While loading the s/c, a rock became fowled in the conveyor chain of the continuous miner. The rock dislodged out of the boom & struck the s/c operator in the left arm. Due to new medical information received on 11/15/13, this incident became reportable on 11/15/2013.

The shield on the longwall pulled in trapping ee heel in between the relay bar and pontoon on the DPT shield.

EE was bolting in the return on 16HG. He was installing supplemental support for the future long wall belt entry. He was lowering the drill head from the roof when his left hand got caught between the drill pot and a piece of drill steel laying in the tray resulting in a laceration to his left hand.

Employee was installing 6 ft torque tension bolts in the 5 right cut. As he pushed the bolt up with the drill pot, the bolt broke causing the bolt to swing out striking him on the top of his left hand.

Roof bolt operator was installing rib bolts along with tensar in the return on a longwall developing section. The bolt man was extracting his steel from the hole when his steel became stuck in the tensar. The operator helper pushed on the tensar and his arm got wrapped in the tensar.

Employee was installing 12' cable bolts when the bolt bent causing it to come out of the drill chuck and struck employee on left wrist.

Turning off compressor and bleeding line. He grabbed wrong valve and a burst of air hit him in the chest, knocked him down.

While installing roof bolts on the 15 Headgate Section #1 entry face, the EE was struck by a roof bolt plate on the palm of his right hand causing a laceration which required 8 sutures to close the wound.

EE was operating the longwall shearer and a piece of rock was thrown from the cutter drum and struck him in the left hand amputating part of his index finger.

EE was operating the longwall shearer. While walking by the shearer a piece of rock approx. 10"w X 30"l X 10"t came out of the pan line and struck EE on his right foot.

Employee was drilling a hole to install a 6 foot torque tension roof bolt when the pusher came off the finisher and the pusher struck him on the right side of his chest.

Employee was operating tail end of shear. A piece of rock flew off the drum striking his right hand.

Employee was drilling a hole for a roof bolt when the drill steel fell out of the hole catching his left middle finger between the drill steel and drill pot.

EE was cutting a flight out of the head drive with cutting torches, when a piece of slag fell into his right ear.

EE was using a hydraulic jack to put supply motor back on track. The jack kicked out striking his right foot.

EE was cutting, a metal beam with a torch when a piece of hot metal went down his shirt. This resulted in burns to his chest.

Employee strained back and right shoulder while drilling.

Victim got his arm caught between winch and monorail.

ROCK FLEW OUT OF CRUSHER STRIKING VICTIM ON THE NOSE CAUSING A CUT AND A POSSIBLE FRACTURE.

While pulling in a shield, a piece of slate. 4" square x 1" thick, fell between the shields and out his neck.

Victim was taking "Dog Bone" out of head drive. Head drive shifted and caught him between motor and pole.

SHEARER ENCOUNTERED A CORE HOLE FULL OF WATER. WHEN CORE HOLE WAS HIT, WATER CAME OUT & HE TRIED TO DODGE THE WATER AND FELL RESULTING IN INJURIES.

INJURED WAS ASSISTING MINER OPERATOR, WHEN A PIECE OF DRAW ROCK FELL FROMT HE ROOF STRIKING HIM ON HIS RIGHT LEG. THIS RESULTED IN A FRACTURE TO HIS RIGHT ANKLE.
