FROM STANDING ON A SLOPED INCLINE FOR AN EXTENDED PERIOD OF TIME WHILE WELDING ON A NOSE ROLLER, THE EMPLOYEE'S HIP BECAME SORE. THE INCIDENT HAPPENED ON 6/22/04. HE TRIED TO WORK THROUGH IT. HIS FIRST MEDICAL TREATMENT WAS ON 7-9-04. ON 7-12-04 HE WENT BACK AND WAS TOLD TO TAKE SOME TIME OFF WORK.

ON 1/26/02 WHILE THE EMPLOYEE WAS BOLTING A BOOM HOLE A PIECE OF ROCK A SIZE OF A GOLF BALL FELL APPROXIMATELY 2 FEET FROM THE ROOF STRIKING EMPLOYEE ON THE NOSE, RESULTING IN A SMALL CUT. ON 8/23/02 THE EMPLOYEE HAD A BLACK SPOT SURGICALLY REMOVED, REQUIRING 2 OR 3 STITCHES.
