While trying to remove buildup on rollers, came into contact with moving part and fractured upper right arm.
Stepped in depression in the stockpile yard causing a jerking in left leg and lower back.
Two employees were removing a walkway from a belt structure prior to installation of beltline. One was inspecting a support angle to determine where it was welded. Another employee cut his support angle at another location allowing the support truss to fall approx 4" striking the first employee's right thumb, causing laceration injury. Required 2 stitches.
TWO EE'S WERE ASSIGNED TO REMOVE A PLATE/DOOR FROM A MACHINE AFTER REMOVING THE BOLTS ONE EE ON THE INSIDE STRUCK THE DOOR TO KNOCK IT LOOSE. THE PLATE FLEW OFF STRIKING THE INJURED TRAPPING HIS LEFT HAND BETWEEN THE PLATE AND THE MACHINEHE PULLED HIS HAND AS IT WAS BEING TRAPPED
INJURED WAS CUTTING ONE PIECE OF STEEL OFF OF ANOTHER PIECE OF STEEL BEAM. ON COMPLETING THE CUT, THE SMALLER PIECE FELL FREE ALLOWING THE BIGGER PIECE TO FALL SOME 2" STRIKING THE LEFT FOOT O F THE INDIVIDUAL. THE BEAM STRUCK THE FOOT JUST BEHIND THE STEEL TOE CAP OF HIS FOOT. THE INJURED REMOVED THE STEEL FROM HIS FOOT AND SOUGHT ASSISTANCE.
THE INJURED PERSON WAS USING A PRY BAR TO HELP SEAT A STEEL LINER. THE PRY BAR WAS NOT SUFFICIENTLY LINED AT EDGE OF THE METAL, RESULTING IN IT SLIPPING OUT FROM THE BAR AND WENT DOWN HIS HAND AND STRUCK A METAL KNIFE BLADE, MASHED LITTLE FINGER. FINGER WAS BROKEN. FINGER DID NOT REQUIRE SETTING THE BONE DID NOT REQUIRE A SPLINT.
AIR ARCING-SLAGE LANDED ON NECK CAUSING JERKING REACTION, ARM GOT CAUGHT ON SHARP STEEL BLADE OF PUGMIXER. TAKEN TO HOSPITAL, ER DR. APPLIED SUTURES TO CLOSE WOUND.
