Showing all 16EE was under the D-10 Dozer welding. Bogie arm strap came loose from the side of the machine causing the bogie arm to swing in a downward position hitting him on the head. Head was cut.
EMPLOYEE SHUT THE DOOR OF THE DRILL ON HIS FINGER & MASHED HIS FINGER. LOST THE FINGERNAIL.
He was pulling on dragline flywheel and pulled a muscle in his left shoulder.
Employee was running dozer, while ripping hard rock. The retainer holding the pin in the ripper tooth broke. He was unaware the tooth was hot; he attempted to drive the pin back into the ripper tooth and when his hand came in contact with the tooth, he burned his left hand.
He was shoveling dirt into blasting hole; strained disk in back and neck.
Climbing on dozer to fuel; slipped and fell to the ground.

EMPLOYEE WAS INSTALLING A SEED GRADER ON A FARM TRACTOR. AS HE WAS CONNECTING A SHAFT, THE POWER TAKE OFF ON THE TRACTOR ENGAGED AND CAUGHT HIS GLOVE, TWISTING HIS RIGHT HAND AND ARM.
EE WAS ATTEMPTING TO HOLD A 3-1/4" NUT WITH A WRENCH WHEN THE NUT TURNED CUTTING HIS LEFT THUMB.
EE WAS ADJUSTING CONVEYOR BELT ON COAL CRUSHER USING A PAIR OF COMEALONGS TO PULL BELT TOGETHER. WHEN HE STARTED TO RELEASE THE COMEALONGS HIS HAND SLIPPED OFF THE HANDLE. THE HANDLE HIT EE AB OVE RT EYE REQUIRING 7 STITCHES.
HE WAS LOADING AN IDLER FROM THE P&H DRAGLINE ONTO A TRUCK; THE IDLER SLIPPED AND CAUGHT HIS FINGER UNDER IT.
HE WAS TRYING TO REMOVE A BEARING WITH A HAMMER AND CHISEL. HE MISSED THE CHISEL WITH THE HAMMER AND HIT HIS LEFT HAND.
