Employee was sorting through pile of rocks with muck bucket placed to the side for employees to sort through scrap and usable material. Rocks ranged from 10-60 pounds. An hour after sorting material, employee had complained of a straining sensation coming from the groin area.
Worker was splitting stone slab with hammer and wedge. Metal wedge broke off and a small sliver pierced EE's Left hand.
Getting a rock off a table the worker slipped and the rock fell on the worker's shoulder

Employee was splitting sandstone using a wedge and hammer. A piece of metal came off of the wedge and lodged in knee.
Injured worker was splitting sandstone with a hammer and chisel. The rock broke and fell on a leg causing an abrasion to leg and a broken ankle.
Lifting rock, it broke in half and one part landed on toe of left foot causing cut and pain
A NEW CONVEYOR WAS BEING ASSEMBLED. The employee went to move a section and the section moved, crushing 2 fingers
EE was moving a piece of flagstone with a partner when it slipped from their grip and fell, striking EE's foot. EE's left foot showed a small contusion on the top of EE's foot, above the steel-toe of EE's boot. X-ray revealed a non-displaced fracture of the second metatarsal. EE is non-weight-bearing on crutches. Modified duty will be assessed on Monday the 18th.
Workers were removing a stone from the cutting table. The rock slipped, and fell. The worker's finger was under the rock and got pinched between the rock and the table. The rock rolled off the finger and continued to the floor where it broke.
He was moving a stone and his finger got caught between the stone and the saw table. His pinky finger was skinned and the nail was damaged.
Individual was operating a Chris Cutter without permission. He had not been trained on the machine and did not know that the table moved. he had his fingers under the rock and when the table rose it pinched his fingers. Two fingers were bruised and / or lacerated.
Employee was using a quarry bar that slipped from under a rock and stressed his wrist.

Lifting a skid plate on heavy equipment. Employee "twisted" to install a bolt and hurt his back.

The worker was using a hammer and wedge to split a stone. The head of the hammer came off the handle and struck the worker's finger. The finger split open and and suffered a tuft fracture.
The worker was using a hammer and wedge to clean rock. He missed his wedge and struck his finger. This removed the nail and some skin from the right index finger.
The Worker was Spliting a Stone with Hammer and wedge. as he was holding the wedge in place, he swung the hammer, missed his mark and hit his Thumb instead.
The worker was bracing a rock to prevent it from jumping when it was cut. The force of the cut proved too great for him to stop and the rock came at him off the end of the table. A pallet that he had placed behind himself earlier kept him from getting clear of the rock so it landed on his foot. Part of the rock landed above the steel toe crushing the foot and breaking one bone.
This is a muscle injury; the worker is not really sure how or when the injury occurred. It was brought to our attention on 8/21/2006. At the time the employee said that the injury had been growing in seriousness for a while. He could not remember when it began or what he was doing at the time.
The miner observed that another piece of equipment was moving with out an operator present & he jumped out of his machine to go arrest the trucks movement. When he jumped out he landed oddly and tweeked his back.
The miner was moving an empty pallet to his work area when he slipped and fell. He caught himself on a sharp rock and cut his hand.
Miner was placing a stone on to a pallet wiht two hands. One hand slipped causing the stone to fall and land on the other hand, breaking one finger.
The miner was splitting a rock with hammer and wedge. A sliver from the wedge struck the miner in the arm. The sliver hit a vein, so we sent him to the doctor to make sure that there was not a serious problem and to make sure the bleeding stopped. The doctor felt that the cut had been well cleaned at the quarry and that the metal still in the arm was not worth removing.
the miner was prying on rock with a pry bar. The rock broke, and released the bar. the shock of this resulted in some strained muscles in the quarriers back.
He was placing a stone on a pallet and the stone slipped and crushed his middle finger on his left hand. The finger was cut badly requiring 4 stiches.
The employee was working with a hammer & wedge clearing rock near a stone face. When the accident occurred his hammer glanced off the stone face in mid swing and was deflected to hit his thumb instead of the wedge. His thumb is broken but appears to be in position.
The employee was walking beside the quarry and stepped on a loose rock and slipped, falling on his shoulder.

THE EMPLOYEE WAS ATTEMPTING TO MOVE A LARGE ROCK FROM THE FORKS ONTO SOME ROLLER TRACK BY BODILY PUSHING THE ROCK. THE ATTENDING PHYSICIAN DOES NOT FORESEE THIS BEING A SERIOUS INJURY
The ee was prying on a rock with a bar. The rock broke unexpected and a piece of rock fell against his ankle.
While moving rocks on rollers for a hydraulic splitter, the ee's right middle finger tip was smashed between 2 2ocks. The finger was cleaned and x-rayed. The last bone in the finger is broken.
WORKER WAS POUNDING WEDGE INTO CRACK IN ROCK TO SPLIT ROCK WHEN A PIECE OF THE ROCK FLEW OFF AND LACERATED HIS SCALP.
WORKER WAS "WALKING" A PIECE OF FLAGSTONE TO ANOTHER LOCATION WHEN ONE CORNER OF STONE SLID BRINGING OTHER CORNER DOWN TO RAKE WORKERS SHIN AND CALF.
WORKER WAS POUNDING A WEDGE INTO A PLUG & FEATHER HOLE CAME OFF AND STRUCK HIM IN THE CHEEK.
