Drill operator noticed hydraulic fluid leak and went to inspect leak. At same time the operator found the leak, the hydraulic hose ruptured spraying hot hydraulic fluid into the face and eyes of the operator. The force of the spray knocked the hard hat from his head and safety glasses from his face resulting in abrasions to his corneas and conjuctivitis in both eyes.

EE WAS STANDING ON A TURNED CUT WHEN A LOADER OPERATOR WHO WAS TRYING TO BACKFILL THE TURNED CUT, ACCIDENTALLY STRUCK THE CUT CAUSING EE TO FALL STRIKING HIS BACK ON THE EDGE OF THE TURNED CUT.

Crane operater was outside his crane attempting to secure a chain hook to saw track when he felt pain in his lower back. EE stated he was pulling and twisting at the same time. Initial doctor examination indicated a strained lumbar. EE out 10-14 days.

EE was using a cold steel wedge to plit/break a quarry block. When EE struck the wedge, a piece of metal from the wedge struck the EE in the forearm causing a muscle laceration and lodging in the top of the EE's forearm. This required minor surgery to be required to remove the metal and repair the damage to the muscle.

Worker was using a "mushroomed" wedge to split a quarry block. When the worker struck the wedge a piece of metal from the wedge struck the worker on the upper forearm just above the inner elbow area causing a laceration which required 3 stitches to close.

THE TOP OF THE WEDGE BEING USED TO BREAK THE QUARRY BLOCK WAS MUSHROOMING AND THE EE INJURED FAILED TO NOTICE, WHICH RESULTED IN A METAL FRAGMENT SPLINTERING FROM THE WEDGE AND PENETRATING THE WORKER'S BOOT AND LODGING IN HIS LEFT FOOT JUST BELOW THE ANKLE. EE WAS TRANSPORTED TO A PROMPT CARE FACILITY WHERE HE RECEIVED A TETANUS SHOT AND SIX STITCHES.

CRANE OPERATOR AND ANOTHER WORKER WERE LIFTING A 6 INCH WATER PUMP FROM A PIT, WHEN THE CABLE HOISTING THE PUMP BROKE CAUSING THE PUMP TO FALLAND STRIKE THE WORKER WITH A GLANCING BLOW TO THE LOWER RIGHT BACK AREA. WORKER SUSTAINED SERIOUS BRUISING AND SWELLING OF THE AFFECTED AREA. WAS TREATED WITH I.V. FLUIDS, X-RAYS, AND RELEASED FROM E.R. LATER THE SAME DAY.
