The employee was walking down a set of stairs when they misjudged the last step and jarred their back.
EE was walking out to diesel tank when he slipped on ice and may have done internal damage to right knee by twisting it. Diagnostic testing will be done on January 5th, 2012 by an orthopedist.
EE was standing on bulk bag scale when he slipped and twisted left ankle.
EE was stacking bags and was breathing hard, taking in more air than the other hands when he passed out. He came to shortly and was rushed to the town clinic and then transported to the hospital by ambulance where it was confirmed he had high levels of carbon monoxide.
While stacking bags on a pallet, the EE felt a pop and subsequent pain in the right shoulder. He went on light duty for 5 days and returned with a full release on 08/12/2011.
EE was climbing ladder and fell back on his rear end when slipping on the second rung. His arms flew back and he contacted a protruding bolt on an elevator with his left forearm, causing a moderately smooth 3 inch long cut. He went to Wyoming Medical Center to have it stitched and returned to work to finish his shift by his own choice.
EE reached with left hand to stop a bag coming down conveyor chute and felt something pop in his left elbow. A visit to doctor on 09/25/2009 revealed a strained tendon and nerve as it was pushed across a bone spur already present at this site. EE was put on light duty until October 1, 2009. No days of lost work are expected.
EE pulled lower back muscles while stacking 50 pound bags onto a pallet. It was determined that she would be better off not doing this kind of work and decided to take a termination. She did not miss work until 03/11/2009.
EE returned from rotator cuff surgery and convalescence today and re-injured his left shoulder again while stacking 50 lb bags onto a pallet. We are awaiting report from doctor.
EE caught foot between pallet and forklift while stacking bags which caused some bruising.
EE was stacking 50 lb bags onto a pallet when he felt pain in left shoulder. The doctor believes it is just torn muscles, not a rotator cuff.
EE lifted the end of a 12" x 17 foot long screw and sustained a hernia in lower left side of stomach. Surgery is scheduled for June 27th and return to work on June 30th. Currently working but on restricted duty for lifting.
EE pulled muscle in upper back just below left shoulder blade while stacking bags of product on a pallet. One day of lost time resulted.

Employee bent down to walk under a kiln dryer and felt a sensation in lower back. An MRI revealed a swollen or herniated disk. Chiropractic treatment has been administered and there has been no lost time for this plant supervisor.
Employee was driving forklift down a hill on uneven dirt road at too high a speed, started to turn right and centrifugal force caused machine to fall over on left side. Seat belt was not in use and he was thrown from machine with left leg extended and protective cage landed on it a few inches above ankle, resulting in compound fracture.Surgeries required to reconstruct leg.
Employee was climbing out of the front-end loader when he stepped on a piece of lignite, causing him to slip. While falling he grabbed the side of the loader and pulled his shoulder, causing an acute shoulder strain.
Employee was stacking 50 lb bags onto pallet when he felt a sharp pain in left arm and shoulder. Doctor believes it is a rotator cuff injury and may need surgery. No lost time as yet. Edoc 154045 corrects the date of injury to 01/04/2005 from 01/01/2005. Surgery was eventually performed on 02/11/2005: 5 days lost time and on light duty until spring - update will follow.
Employee exited loader, failed to use the bottom step and heard and felt a loud pop in right knee. This caused him to fall down and to do some limping for awhile. A visit to the doctor the next day indicated he compressed the cartilage.
THE EMPLOYEE WAS STACKING 50 POUND BAGS ON A PALLET WHEN HE FELT A SUDDEN PAIN IN HIS UPPER BACK BETWEEN THE SHOULDER BLADES. HE VISITED THE DOCTOR, MISSED ONE DAY OF WORK AND IS ON LIGHT DUTY FOR 5 DAYS.
THE EMPLOYEE TWISTED LEFT KNEE WHILE STEPPING DOWN FROM A FRONT-END LOADER. THE KNEE SWELLED, WAS OBSERVED FOR A PERIOD OF TIME, AND SUBSEQUENTLY OPERATED ON FOR A TORN CARTILAGE ON DECEMBER 1 6, 2003. HE WAS PLANT OPERATOR AT TIME OF ACCIDENT AND PLANT MANAGER AT TIME OF SURGERY.
EE WAS WRAPPING A CHAIN AROUND ONE END OF A SCREW WHEN A FELLOW EE LIFTED THE OTHER END, CAUSING THE SCREW TO ROLL TO THE SIDE AND ONTO EE'S RIGHT HAND. X-RAYS AT THE DR'S OFFICE REVEALED NO B ROKEN BONES OR ABNORMALITIES. EE MISSED 3 DAYS OF WORK.
