Employee was striking a chisel while carving a piece of stone. A small chip broke off the chisel, resulting in a minor laceration on left thumb. The chip did not exit, remaining in thumb area. Injury 5/17 - continued working. Complications caused employee to seek medical attention on 5/28/13.

While building a pallet of stone, a stone slid off the pile and smashed employee's left thumb nail. Initially it was a bruised nail, after a follow up exam, a fracture was found to the left thumb. More follow up exams to follow.

AUGUST 6, 2004 WAS A WARM, DRY DAY. A ROCK SLID OFF A PILE APPROXIMATELY TWO FEET AND HAD BROKEN THE RIGHT INDEX FINGER OF THE EMPLOYEE.
