Employee alleges he had a work related injury to his left elbow 12 years ago and he declined surgery at that time. He reported pain in his elbow and received first aid diagnostic treatment on 12-31-09. On 2/8/2010 he was placed on restrictions that prevented him from doing his regular job at which time this became reportable.

Showing 50 of 138Employee alleges he strained his right groin area as he slipped while sweeping clinker and did a half spllit. He failed to report this accident until 12-15-09 at 8:50 AM. He was sent to the Industrial Clinic and was placed on restricted work duty. This alleged incident is being investigated as to whether or not it is work related or an incident that may have happened at home.
Employee was attempting to light a torch and it flashed back and he sustained burns to his face, ear, neck, part of his arms and part of his hands.
Employee was using a pry bar to hit an air activated rail car door that did not open. He was crouched down so he could pry it open, but there wasn't anywhere he could pry it. As he was crouched with the pry bar in his hand, the door automatically opened, striking the pry bar which hit the employee on the forehead.
Employee did not have an accident, but came in the following morning and said his leg began hurting at night. He is alleging that his left leg was strained from doing his regular job tasks.
Employee was pulling bags from one of the baghouses and felt soreness in his left wrist and left lower arm.
Employee was working at the bottom of a cone crusher when a rock rolled down rendering him unconscious. Employee was air evacuated to a trauma center with head trauma/concussion.
A 23 yr old was driving on the road toward the entrance. Was driving under the influence of alcohol. He sustained head injuries, multiple body contusions and abrasions. Was air lifted to medical facility. Prior to being transported was placed under arrest, CHP Report# 985017113. Was not an ee had no business at the facility. He thought he was driving in a different city.
As employee was replacing a 2 inch water line to the GPH3TAL, another water line ruptured with hot water and steam and hit employee in the face. Employee had his safety glasses on but sustained a corneal abrasion to the left eye. A piece of metal was removed from his left eye.
Employee alleges he aggravated a pre-existing non-work related injury to his right shoulder on 7-17-09 while setting a return roller into a conveyor frame. Employee felt okay until 7-24-09 at which time he requested medical treatment. Employee was placed on restricted work duty for a strain to his right shoulder.
Employee was injured on 6-26-09, but did not report this injury to his Supervisor until 6-29-09. Employee was turning the bottom cap of an airslide in the basement of Group 4 Silos, 26, 28, 3 0 (airslide to ME5 elevator). As he was turning the cap, it stopped while he was still applying pressure, and his thumb bent backwards.
Employee was picking up a 1 1/2 round stock and felt a pain in his lower back.
INJURY ALLEGES HE SUSTAINED AN INJURY TO RIGHT SHOULDER DUE TO POSSIBLE WORK RELATED ACTIVITY. HE HAD BEEN UNDER THE CARE OF A PERSONAL PHYS. FOR MONTHS TREATING FOR SHOULDER AND BACK INJURY. HE HAS BEEN PLACED ON LIGHT DUTY WORK. THE COMPANY IS INVESTIGATING THIS ALLEGATION BUT WILL REPORT AS PER REGULATORY REQUIRMENT TO AVOID PENALTY.
EE a Mine Engineer, was surveying the pit using a GPS system. He walked out 6' from the edge of the bench when the ground sloughed out from under him.
Grinding commutator on 700HP DC Motor. Glove got caught between commutator and brush holder. Pulled left thumb into gap between commutator and brush holder resulting in laceration/abbrasion to thumb
A fatal accident occurred on mine property. Three off-road motorcycle drivers trespassed onto private property. One of the drivers lost control of his motorcycle and went over an embankment. The motorcycle driver was pronounced dead at the scene. The motorcycle drivers were exceeding the posted speed limit.
Employee driving truck somehow dust got into the truck and got under the safety glasses.
Was using water blaster cleaning cement riser. Chunk fell from side of riser hitting the tip of the water blaster nozzle.
