EE WAS WORKING IN SHOP ON NEW TRAY FOR THE #1 FEEDER. TRAY WAS SET ON A FRAME. EE DROPPED A TOOL. WHEN EE BENT OVER TO PICK UP TOOL, HE SET HIS RIGHT HAND ON FRAME FOR BALANCE. THE NEW TRAY SLID FORWARD AND DOWN, CUTTING EE'S MIDDLE FINGER ON RIGHT HAND. EE WAS WEARING LEATHER WORK GLOVES WHEN THIS OCCURRED.

Showing all 5On Thurs afternoon ee & a co-worker were completing repairs to loader. Ee stated he felt a slight pain in shoulder. Ee did not work on Friday. Monday ee asked to have his shoulder looked at. He was checking the torque on bolt to insure tightness & strained his right shoulder, on Thursday 6/17/04.
EE WAS INSTRUCTED TO TRIM BRUSH & WEED ALONG A STAIRWAY TO LOWER GARAGE. HE EVIDENTALLY CAME IN CONTACT WITH POISON IVY. HE REPORTED THIS ON WED. 7/15/03 - THE POISON IVY SPREAD, WENT TO DR ON 7/17/03.
WHILE WALKING ACROSS A LADDER BEING USED AS A BRIDGE TO A CRUSHER, EE FELL ABOUT 2 1/2'-3' TO THE GROUND. AFTER ACCIDENT, EE DECLINE MEDICAL ATTENTION. TWO DAYS LATER, EE CALLED IN SICK. EE'S DR. NOTIFIED EMPLOYER THAT EE WOULD NOT BE ABLE TO WORK UNTIL REVALUATION ON 3-23-02.
EE AND COWORKER WERE REPLACING CATWALK BOARDS. EE WAS SITTING ON CONVEYOR #6 BELT. THIS CONVEYOR NOT LOCKED OUT, TAGGED OUT. CONVEYOR #6 WAS INADVERTENTLY ENERGIZED. AND EE WAS PULLED UP TO A BEWLT SUPPORT. THIS ACTION PULLED EE AROUND & THE SUPPORT HIT HIM IN THE BACK. STRAINING A PRE-EXISTING BACK CONDITION.
