While eating employee got something in her right eye. Other EE's washed it out and EE still complained about pain in her eye. She was taken to Concentra where her eye was determined to have been chemically burned.

After completing a series of tasks including supersack bagging, driving forklift, collecting samples, and housekeeping, employee was climbing a flight of stairs when he reported feeling pain in his lower back.

Employee was returning to Control Room, when he stopped to talked to another employee. A yellow jacket landed on his arm just above the elbow and stung him. He is allergic to insect stings and his arm began to swell. He was taken to medical center for evaluation.

During a rainstorm, the area around the Bathroom was flooded. Employee had to jump to get up on the metal walkway going to the bathroom stairs. When he jumped he reported feeling a sharp pain in his Right hip. This injury was originally reported on 4/21/06 as a near miss, employee reported feeling pain on and off and requested medical attention 7/17/2006.
