Condensate sprayed on employee
EE was on a scaffold in 24 area and squatted down to move into position to replace the old valve with a spool as he turned to face the pipe he twisted his right knee and felt it pop. Strain-right knee
While walking east on the main roadway between Fac. 45 and Fac. 50 a gust of wind blew particles in EE's right eye.
Employee was hosing down under 5-1 heater under the grating. EE was squatting down to hose under the grating, When employee went to get up, he felt a pain on the inside of his right knee as he pushed him self up. Torn ligament right knee.
On 03/28/13, EE reported to onsite medical dept. for groin pain. EE sent out for non-occ bi-lateral hernia. EE could not give a time/date of an injury. On 04/19/13, EE returned to onsite medical facility claiming that the hernia was work related, still could not give date/time. On 05/15/13, EE brought document from physician proclaiming that the hernia was work related.
EE squatted down to go under some low hanging piping. When he straightened up he felt a pain in his lower back. EE placed on restricted duty on 12/10/12.
Original diagnosis was a right thumb sprain. A follow-up doctors visit on 03/05/2012,resulted in a diagnosis and treatment plan that placed on EE's right hand in a hard splint and restrictions placed on use of same.
EE entered a mine site operator control room at approximately 0630 hours. He stated to the Lead Operator that he was not feeling well. EE sat down,started displaying signs and symptoms of physical distress and collapsed. CPR was initiated and the mine site EMS Team was mobilized. EE was transported to a local emergency room for treatment. EE did not survive.
Employee sprained his left knee reporting to work on 11-1-11 while riding his bicycle. Employee said that he slowly started feeling discomfort in his left knee after dismounting the bicycle.
When EE removed his mono-goggles to clean them, a foreign object blew into his right eye. EE went to the mine medical facility. EMT was not successful in flushing the foreign object from the eye. EE was transport to a local hospital where the foreign object was removed. EE prescribed an antibiotic making this a recordable injury. No lost work days or restricted duty.
EE was performing routine duties when he sustained a second degree burn on his left lower leg. EE returned to normal duty without restrictions or lost work days on the following day.
Initially diagnosed as a right wrist sprain and returned to full duty on 7/19/2010. EE continued to complain about numbness and tingling in right hand. X-ray has revealed an avulsion fracture of the right wrist. EE placed on restricted duty.
EE stung by a bee on right side of neck below the ear. Released full duty on 4/28/2010
EE first stated that on 11/20/09, EE woke up with both shoulders sore. Several days later, EE changed his statement and said that the soreness was due to work he did on 11/05/2009. On 01/13/2010, EE called in and stated that he had gone to his doctor and his doctor took him off work. Company is contesting this injury.
