An employee was preparing to turn to move the continuous miner when they noticed a pain in their left knee that ultimately was discovered to be a torn meniscus.

Employee was reaching across the bolter and felt a pain in their left arm. Possible strained/tore soft tissue?

None. Employee was found unresponsive at their desk with no pulse, soon after declared deceased by medical professionals.

The mechanic stated: was attempting to step up on a handrail to remove a chain on the drill. When EE raised EE's leg EE felt this knee pop. This resulted in a partially torn tendon.

Employee was at 6 BK on Old 1-C Panel lacing 1000' of belt. When EE reached down to pick up the belt clamps, EE felt sharp pain in their back.

The employee had crossed the belt to clean the #2A tailpiece. While walking toward the tailpiece, EE felt a "pop" in the right knee. Medical examinations revealed a torn anterior cruciate ligament that had been torn previously while playing high school football.

The employee was attempting to crawl from the #9 entry to the #10 entry through the bolted but uncleaned crosscut to work on tracking and communication. EE felt a pop in lower back resulting in a strain.

The employee was underground and preparing to load used conveyor belt structure in a scoop bucket. The employee pulled the scoop up next to the conveyor structure and exited the scoop. At this point they felt pain in the middle and lower parts of their back. Medical examinations reveal extensive arthritis in the employee's back.

Employee was walking in the back area of the mine office when EE observed a black snake by the trash can. The employee bent over to pick up the black snake when EE was bitten on EE's right hand between EE's pointer finger and index finger. The employee did not miss any work and both agencies were notified by calling the hotline as a safety precaution.

Employee was jacking up a flat car and as EE was in the motion of standing up EE felt a pop in the right knee. Employee then went to Raleigh General after the shift ended and it was determined EE needed an MRI. MRI results showed a torn meniscus on 5-20-16. Note: Injury report was received on 5-20-16.

The employee stood up from the kneeling position and felt a pain in his right knee. Employee was working on a rubber tired ride near the bottom diesel shop area. Employee finished his shift and returned to work. On 7/13/2015 doctors put him on restricted duty.

He was working on a carrier haulage system when he bent over and his back went out.

Employee was walking through a check curtain and the wind blew the curtain into his face, causing debris to blow into his eye.

Employee bent down on one knee to take an air reading and felt pain in his knee. He went to the phone & went outside & was transported to the hospital.

The employee received a new knee brace on 10/2/2012 for pain to his right knee. The employee never reported an injury to the company and is no longer employed. This is to report medical treatment only. The company believes this injury relates to an injury that occurred on 6/16/2012 that the company does not believe occurred at the mine.

The employee stated he pulled groin muscle while getting up from his knees. The company does not believe this injury occurred at the mine. This is to comply with citation 9051012.

Employee was standing in the doorway when a gust of wind blew the door shut hitting the employee in the forehead. This caused a laceration that required sutures. This is to report medical treatment NO days were missed.

Employee reported on 6/18/2012 he had been injured at the mine but did not report it. An investigation was conducted and the company does not believe an injury occurred at the mine. The report is to comply and abate citation 9051005 issued 8/27/2014.

Employee passed out as he exited truck.Worker saw him covered with mud.No injury occurred.No exposure to toxic hazards.Went to hospital for heart tests found nothing.Was admitted due to infection & given antibiotics.Was taking antibiotics prior to incident.Reporting this as occupational illness MSHA says we must.We do not agree this is reportable would like it removed from record.

A argument ensued over how to do something in the mine. One miner grabbed the mining belt of the other and threw him down jumping on him in the process. The miner who was grabbed and thrown down suffered a fracture to his right ankle.
