Employee got in truck and started to leave going to Dukes Mine. Minutes later EE began to feel jittery and dizzy and turned around. EE's coworker went to check on EE and saw that the EE was pale in color and nauseated. The decision was made to take EE to the ER.

IW and co-worker were returning from lunch and headed back to maintenance shop on the golfcart when the IW collapsed and became unresponsive. The Co-worker called 911 and started CPR. The Co-worker was immediately assisted by other co-workers with first aid/CPR efforts until EMS arrived. EMS arrived and transported IW to a Medical Center.

The employee operated a loader that had not been operated in about 3 months. The employee noticed a spider bite on right arm when employee was showering after work. Bite was deemed to be from a brown recluse.

The employee was trying to loosen a manway bolt on a tankcar when they felt a sharp pain in groin area. The employee did not report the incident until 10/19/20 but did not request to see a doctor until the 20th. EE missed the first shift on 10/23/20 but there are conflicting diagnoses varying from a mass to a hernia.
