EE was signaled to tram s/c forward by operator twice & didn't respond. Operator approached ee & observed ee slumped in seat. Not able to arouse ee operator called for boss/met boss checked for a pulse & none found removed from car & started CPR transported to surface while performing CPR. Continued until ambulance personnel took over corner pronounced dead at mine.

AS PER I NAME HE SAW HIS DOCTOR ON 04-12-91 AS THE PAIN IN H IS LOWER BACK WAS WORSE I NAME STATED THAT THIS WAS RELATED TO THE INJURY WHICH OCCURRED ON 00-12-88 & THAT THERE WAS NO NEW ACCIDE NT WHICH CONTIBUTED TO THIS (PLEAST NOTE THAT THE MINES WAS INACTIVE 03-30-91-04-15 -91)
EE STATED THAT HE WAS DOWN ON HIS KNEES SAWING A WATER LINE; AND AS HE GOT UP, HIS RIGHT KNEE POPPED.
I.NAME CALLED ON 12-30-88 REPORTING HE HAD SEEN DR FOR INJURY TO RIGHT KNEE.ACCIDENT OCCURRED SOMETIME LAST SUMMER HOWEVER HE WASNT SURE OF DATE OR TIME.NOT SURE HOW IT HAPPENED MAYBE HIT IT O N MINER OR TWISTED IT.ALSO NOT SURE WHETHER HE REPORTED IT TO FOREMAN.
