On 07/30/2021 a contractor did not report home & sheriff was notified. On 07/31/2021 the contractor was found unresponsive by county deputy. An ambulance and coroner were summoned & the contractor was pronounced deceased on site at 5:45am.

Miner was walking on Dragline Bench to Pickup for shift change. High Wind Event 30 to 40 mph gusts. Got a piece of dirt in Right Eye. Miner thought EE cleared it. Next morning dirt needed to be removed and treated with Tobradex Opthalmic.

Miner Welder was Air Arching both his welding helmet and safety glasses were being used, a gust of wind came up and the miner felt he got something in his Rt.Eye. He tried to clear it with the eye wash station. Later the miner went to Dr.s office to have dirt removed.

EMPLOYEE WAS MOVING DRAGLINE POWER CABLE WITH A DEUTZ-ALLIS CABLE TRACTOR WHEN THE WIND BLEW THE HOOD UP AND BACK, STRIKING THE LEFT SIDE OF THE HEAD. THE WIND WAS 25-30 MPH AND THE HOOD LATCH ES FAILED.

EMPLOYEE HAD FINISHED A REAPIR JOB ON A DRAGLINE AND WENT TO THE LOCKER ROOM, CLEANED UP AND WA S WALKING ACROSS THE LOCKER ROOM WHEN HE FELT HIS KNEE POP AND TWIST. EMPLOYEE STATED HE NEVER S LIPPED OR DID NOT INJURE HIS KNEE WHIEL WORKING.

EE WAS FUELING A SCRAPER WHEN HE FELT CHEST PAIN AND PAIN IN HIS ARM. HE CALLED HIS SUPERVISOR AND ASKED TO BE TAKEN HOME. SUPERVIOSOR TOOK HIM HOME AND MADE SURE THE EE S WIFE TOOK HIM TO THE DR AND GET CHECKED OVER. HE WAS PUT IN A HOSPITAL AND HAD TRIPPLE BY PASS SURGERY JUNE 25, 1990.

EMPL WAS AT 757 DRAGLINE HELPING TO CHANGE DRAG SOCKETS WHICH DOES INCLUE PIN REMOVING & INSERTING WITH SLEDGE HAMMER. EMPL DOEN'T RECALL SPECIFIC INCIDENT WHICH CAUSED FOREIGN OBJECT TO ENTER EYE. THERE WAS STRONG WIND GUST UP TO 50 MPH. EMPL WENT TO DR TO HAVE OBJECT REMOVED FROM EYE.
