Showing all 7Employee was using a 1 inch impact to remove bolts from a sprocket segment on a D11 dozer. EE stood and stretched after removing the bolts. EE then felt EE's back crack, causing a strain in EE's upper back. EE was treated with First Aid measures until 8/29/2022 when EE was prescribed physical therapy.
Operator was attempting to access the cab of CAT 345 track hoe. Employee pushed off with leg when they felt a pop and discomfort in left knee. Employee was transferred to the hospital for observation. Employee was sent to an Orthopedic surgeon. Was released to return to work on restricted duty on 12/18/2020.

WALKING PAST BALL MILL-WHICH WAS BEING WASHED-A DROP OF WATER WAS ABLE TO PASS AROUND HIS SAFETYR GLASSES LEAVING PARTICLES IN HIS EYE.
