EE was removing an extension cord from the cyclone. Dust blew into right eye around safety glasses. EE washed the eye out with eye wash solution but said that the eye was still feeling like something was in it.

Employee tested positive for covid on 12/1/20. I was notified by employee that was admitted to the hospital on 12/5/2020. I reported it to MSHA to err on the side of caution. Employee is not aware of how or when they got the virus.

EE was removing blower, when he squatted down & felt right knee pop. EE continued working for following days until dr. prescribed no climbing restriction.

Diagnosis from the DR. said to be an Acute reaction from lack of vitamins, sleep, and dehydration caused his body to go into convulsions. the employee apparently was going through alcohol withdrawals while he was sustaining his attempt for sobriety .

EE reported (3 days later) back discomfort, and was unsure of activity that caused discomfort. Discomfort did not restrict EE job assignments until 2/4/13.

National Cement was notified by rescue services, that an individual jumped off of highwall of non-active flooded quarry. The individual did not resurface and recovery actions were initiated. Immediately called MSHA hotline, and explained that the individual was trespassing on property, and was not a work related fatality. MSHA inspector arrived and conducted investigation.

Employee was stung by a bee on 8/9/12 and reported the incident the following morning after some swelling had occurred overnight. He was taken to the doctor and then placed on light duty activity on 8/10/2012.

EE felt pain in abdomen while at work, but when reported, was not sure of activity that caused discomfort. Physician diagnosed as hernia, and EE underwent surgery on 8/16/12.

About 12 noon Injury,other ee and another ee were adjusting the tarps on top of the outside clinker pile. Other ee and Injury were working together trying sand bags to the tarps to weight them. As other ee step back a gust of wind came up lifting the tarp throwing injury up in the air about 8 feet. Other ee dove pulling injury free of the tarp.

Employee was lying in the bottom of a motor and due to the awkward position he was in he strained his back. Employee did not report the accident until 2/19/08.

EE took medical leave to have disc surgery. While awaiting surgery, employee filed for work comp. and received doctor statement of work related cause of medical condition. There has been no specific accident or date related to this condition.

EE reports feeling sharp pain on neck when positioned on side, vacuuming on 31BC01 conveyor. On 3/24, employee reported small welt on back of neck, sent for med eval. On 3/27, physician placed on TTD. First lost workday 3/25. Probable insect bite or sting.

2/8, mgmt first notified by unsigned reports that EE experienced back/neck pain on 2/6 & was consulting personal physician. 2/10 EE reported by phone that condition was result of mental stress. 2/10 mgmt received diagnosis "Alleged Stress Cl[aim]". 2/11 EE reported condition related to concern over disciplinary issue and other interpersonal relations--under investigation

EE alleges back pain after turning around to get off work stool in shipping silo operator's booth. EE was offered medical treatment after reporting the condition, but refused until she requested medical attention with her personal physician on 5/2/04. She received medical evaluation on 5/3/04 and began loosing time from work on that date.
