As part of a blast crew IP was stepping over a stemming pile and twisted their lower back.
Employee started having pain in side and was struggling to get breath. Due to an underlying health condition 911 was called. Due to EE's medical history EMS decided to transport EE to the hospital for assessment. EE was released from the hospital on 11/2, and tested positive for COVID-19 on the subsequent day.
EE OPENED ACCESS DOOR OF THE PRIMARY CRUSHED. WHILE ATTEMPTING TO CLEAN ROCKS OUT, THE WIND BLEW THE DOOR CLOSED STRIKING THE EE ON THE LEG.
WORKING ON DRIVE SHEEVE, OUT OF MANLIFT, MAYBE 6' OFF OF GROUND, OPENED HINGED DOOR TO SHEER WIND GUST BLEW DOOR SHUT STRIKING EE IN THE BACK OF THE HEAD PUSHING HEAD INTO SHEEVE, RECEIVED CUT LEFT SIDE OF FACE APPROX. 1/2" FROM CORNER OF LEFT EYE, RECEIVED 4 STITCHES AT COLUMBUS COMMUNITY HOSPITAL.

EMPLOYEE WAS REMOVING DRIED MATERIAL TO UNCOVER GREASE FITTING. WIND WAS BLOWING AND SOMETHING FOREIGN ENTERED RIGHT EYE.
RIGHT FRONT CABLE ON DREDGE HOOKED ON HEAD WIND, BLEW BACK OF DREDGE AND RELEASED, CABLE SNAPPED INTO THE LEFT LOWER LEG, KNOCKED OFF FEET.
