Showing all 6EE was entering the operators cab of a CAT 982M wheel loader. As ee went from the door way of the cab to positioning body in front of the operators seat, ee felt a pop in lower back. Then when ee sat down, ee had severe pain in right hip (sciatic nerve pain).
He was lying on the belt under the Jaw crusher to tighten the toggle springs. He set the tools down to roll over to the other side. He noticed a pain in his back as he rolled over. He finished the shift. He reported to work at our asphalt plant on 5-21, 5-22, and 5-24. On 5-25-10 He said that he was experiencing back pain and was unable to perform his regular work.
While cleaning frozen dirt from some bolts to be taken out, he brushed some dirt down in front of him & the wind blew something into his eye.
Results of an audiogram showed that employee had hearing loss determined to be reportable.
