I NAME EMPLOYED BY TEXAS TALC COMPLAINED OF NUBNESS IN HIS LEGS ON 1-10-89 I NAME WENT TO A DR ON 1-12-89 BECAUSE THENUMBNESS PERSISTED AND THERE WAS GRADULE INCREASE OF PAIN IN HIS LEGS THE EMPL DID NOT RETURN TO WORK THE DR STATED THAT I NAME HAS A DAMAGED DISK IN HIS LOWER BACK THERE WAS NO ACCIDENT OR INCIDENT TO DETERMINE WHEN THIS CONDITION HAPPENED OR WHAT MIGHT HAVE CAUSED
