On the morning of 3/7/2018 the employee reported that the employee thought something was in eye. Although employee was not sure when the incident occurred employee believed it happened while working in the Shaft on 3/06/2018. The employee was treated and released by a local eye doctor.

Showing all 4The crew was in the process untying and moving the canopy when a sudden gust of wind violently blew the canopy over. The frame of the canopy struck EE in the left shoulder. He was taken to urgent care in gold canyon by a supervisor. EE was treated and released to return to work on light duty. He was diagnosed with a dislocated shoulder and a cracked rib.
It was a windy day and saw dust and debris went into left eye of employee. Employee was wearing safety glasses at the time of incident. Eye had abrasions on it.
Worker was moving drill cores in the core warehouse and felt pain to his right leg. The next morning the leg was swollen. Employee went to the hospital and it was identified as an insect bite, was prescribed antibiotics for the reaction and returned to work full duty.
