An employee was installing socks into a dust collector. He was inside the dust collector with his head sticking out of the door opening. Wind blew the door over and the door struck him on top of the hard hat, causing a pain in his back. The employee went to the doctor where he received a prescription as treatment.

The swelling did not subside so on 10/14/15 the employee went to the doctor where he received a prescription as treatment.
