Showing all 4Two tram operators were operating trams headed toward 9 Vent bottom when they seen the victim laying next to the track rail. He was unresponsive and had no pulse. CPR started immediately. Cause of death was probably heart attack. The victim had no signs of injury on his body.
Employee was working at a desk in the office at the Horn Mt. Portal of the Buchanan 1 Mine on 06-29-14. At about 8:30 AM she started feeling sick and left at 10:30 AM. She went to the doctor and it was determined that she had a urinary track infection. She returned to work on her next scheduled day to work which was 07-01-14.
On 3/29/14 said his left arm was numb and he was having chest pains. Supervisor reported to site. EE refused an ambulance to be called and said it was not job related. Supervisor observed EE before he departed site by driving. His mother was going to meet him and take him to the ER. He was asked to provide a medical excuse. EE is not schedule to RTW until 4/1/14/. No lost time.
