Showing all 20EE was painting the underneath steel structure of a lowboy trailer from a rolling creeper. When the EE stood up, they felt discomfort in right knee. The EE applied self care measures until they were evaluated by a panel provider on April 14, 2022 when the injury became reportable.
Panel operator was ascending the catwalk to screen #4 at the Tertiary Plant and felt a pop in right knee. No initial medical treatment was received. An MRI was done on October 7 which revealed a torn medical meniscus. Ortho eval on October 15 prescribes physical therapy with no restrictions.
Two employees were mounting a storage cabinet to a wall inside the tool trailer. One employee turned to pick up an impact drive on the work bench and felt discomfort in lower back and hip.
Employee was on knees working on a water pump at the wash-out. When they stood up, the right knee popped. Employee continued working thru the day but the next day it was swollen. The doctor said the ligaments were OK and gave employee a Cortisone injection.
OPERATING CONTROL BOOTH FOR PLANT DIRT GOT IN EYE.
A 19 year old man died while swimming in the quarry. Police report states he went underwater and never resurfaced. The quarry is a restricted area with no trespassing signs posted. Fire department reported this incident to MSHA but did not notify Eastern Industries. This person was not an employee of the company.
employee worked 7PM to 7AM shift. Before dark his bright vest was covered with little black bugs. As the night progressed, he started getting bites on him. By morning he had several bites, a welt on his arm, and his lip was swollen up.
An unknown trespasser was swimming in the closed, abandoned quarry section of property with several friends when he entered the water at approximately 2:45 pm and did not resurface. His friends called 911 and his body was recovered at approximately 4:00 pm by local water rescue team divers. State police were on scene but did not contact the mine owners.
A miner was observed collapsing next to his service vehicle while changing his work boots after his shift was completed. EMS was called and transported the miner to a local emergency facility. The miner died of natural causes several days later in the hospital.
The EE called his supervisor to complain of chest pains. The supervisor immediately told the EE to shut down the equipment he was operating and that he (the super) was on his way. The EE was finally convinced to go to the hospital after speaking with his supervisor and his physician. He is currently treating for hypertension.
The EE was exiting the track hoe when the door blew shut causing a minor wound the his R leg just below the knee area. The injury exacerbated an existing condition causing infection. The employee didn't tell the site supervisor until 3/15 and it was not reported until 3/16.
WHILE EMPLOYEE WAS PREPPING HIS TRUCK FOR SERVICE, ANOTHER EMPLOYEE WHO HAD A CONFLICT WITH HIM, APPROACHED AND STRUCK HIM IN THE HEAD WITH HIS LUNCH BOX CAUSING A SEVERE LACERATION ON THE RIGHT SIDE OF HIS HEAD.
Tying steel column and something flew into his left eye.
While running the bulldozer, the employee developed chest pains which ultimately were diagnosed as a heart attack.
Employee was getting up and twisted lower back area.
MASSIVE HEART ATTACK
WIND WAS BLOWING, DUST BLEW IN HIS EYE.
PERFORMING MAINTENANCE ON PLANT THREE CONVEYOR. EE BLEW DUST OFF OF A BOLT AND DUST/DEBRIS BLEW INTO HIS RIGHT EYE CAUSING IT TO BE SCRATCHED. CONVEYED TO HOSPITAL. DR. PRESCRIBE AN OINTMENT A ND GAVE HIM AN EYE PATCH TO BE WORN FOR 24 HRS.
AFTER JACK HAMMERING EE STARTED HAVING PAINS IN HIS CHEST, BACK, AND LEFT ARM. ON 10-27-00 STARTED HAVING PAIN AGAI WENT TO E.R. AND WAS ADMITTED.
